The Cardiac Medicine Intermediate Care Unit is a 24-bed unit dedicated to delivering intermediate-level care to adult and geriatric patients with complex cardiac conditions and co-morbidities. Our patient population commonly includes those experiencing: Heart failure Dysrhythmias Myocardial infarctions (heart attacks) Pre- and post-cardiac or vascular procedures Medical conditions with significant cardiac history Responsibilities: 1. Assist physicians and other health care providers with routine examinations and procedures, as well as procurement of equipment and supplies during emergency situations. 2. In the psychiatric setting, aids patients in reality orientation, participates in behavior modification plans/therapy, provides informal support therapy to patients/families, provides verbal de-escalation of potentially aggressive patients, aids in the physical management of aggressive/out-of-control patients, facilitates age appropriate play for children, and monitors the therapeutic milieu activities on assigned unit to ensure that emotional and safety needs of patients are met. 3. Observes, collects, and documents patient information obtained while providing care and promptly report significant changes in the patient's condition to a registered nurse or physician. 4. Provides assistance to patients and their families during admission to the clinical area, orientation to the hospital/clinic/home health experience, and discharge 5. Provides care/ treatments to patients as outlined in plan of care including, but not limited to, the measurement of vital signs, height and weight, specimen collection, skin preps/scrubs, catheter care, capillary blood glucose checks, simple wound care, range of motion exercises, postural drainage, and supportive 11 patient and family interactions. 6. Provides direct personal care to patients as outlined in plan of care for routine activities of daily living such as bathing, dressing, feeding and assisting with ambulation. 7. In this position, they will provide comprehensive, patient-centered care across the continuum of serious illness, supporting patients in both palliative and hospice settings. The role includes a blend of hospice and palliative responsibilities with flexibility based on patient census and program needs. The Nurse Practitioner will deliver advanced clinical care, perform patient management visits and hospice Face-to-Face (F2F) encounters for eligibility certification and recertification, and partner closely with physicians and interdisciplinary teams to ensure high-quality, compliant, and compassionate care. Utilize Local Coverage Determination (LCD) criteria to support hospice eligibility and recertification. Document clinical findings accurately and attest F2F encounters in the medical record in compliance with regulatory standards. Communicate findings, treatment plans, diagnostics, and medication reviews with IDG/IDT. Provide clinical recommendations to the interdisciplinary group (IDG) based on assessment findings. Ensure alignment with Medicare, Medicaid, and accrediting body requirements (e.g., ACHC). Notify appropriate team members promptly of any barriers to completing scheduled visits. Provide patient care and visit coverage across multiple provider numbers, as needed and approved. Maintain comprehensive0 knowledge in the field of hospice and palliative care. Collaboration & Communication Partner effectively with physicians, nurses, social workers, chaplains, and other IDG/IDT members.
