Provider will see patients in various venues of care such as personal residence, assisted living facilities, and skilled nursing facilities. WHAT WE NEED : Ideal candidate will have 2 to 5 years of Palliative, Home Health or Primary Care experience, with a focus on clinical excellence and patient satisfaction.
