Summary The Oncology Nurse Navigator 2 is a registered nurse (RN) who works with physician practices and multi-disciplinary teams to promote the effective utilization of services and coordination of care for patients within the Cancer Center service line. The Oncology Nurse Navigator 2 contributes to the assessment of patients' psychosocial, financial and clinical needs throughout the continuum of care ensuring high quality care while supporting quality metrics of the Cancer Center and Rush University Medical Center The Oncology Nurse Navigator 2 functions as a liaison between patients, physician practices, the hospital, and the community performing functions that meet the core competencies of oncology navigation; Care Coordination, Communication, Education, and Professional Role/Care as well as the domains of the Profession Practice Model; Relationships and Caring, Evidence Based Practice, Technical Expertise, Critical Thinking and Leadership. The Oncology Nurse Navigator 2 care may be focused in medical oncology, hematology, stem cell transplant, gyne-oncology, surgery, radiation oncology, cancer screening or supporting multi-disciplinary clinics. Assists providers with procedures utilizing appropriate technique. Assesses patient and family learning style to provide ongoing education regarding diagnosis, staging, treatment, side effects, and self-care. Responsibilities Maintains and ensures compliance of all policies and procedures that guide and support the assigned services. These include, but are not limited to Joint Commission requirements, federal and state regulations, regulatory standards, etc. Provides consultation in specialty area to all levels of health care providers throughout the organization, affiliated schools of nursing, and the community. Offers guidance and emotional support to patients regarding decisions on follow-up and treatment plans. Arranges appointments with patients, reviews test preparation procedures, and assesses potential patient barriers to ensure the patient completes testing process. Contacts patients via phone calls, emails and letters to remind them about appointments, assists in scheduling/rescheduling appointments and answers questions about testing and treatment. Performs follow-up phone calls to patients after appointments as indicated. Works with participating sites to develop mechanisms for the sites to share the patient data (for those requiring follow-up) in a manner that utilizes their existing data and optimizes their time and effort. Completes all necessary electronic records and paperwork regarding patient interactions and appointments. Develops and maintains relationships with community leaders, community-based organizations, faith-based organizations, health institutions, schools, daycares, etc. Attends community-based events, including health fairs, support group meetings etc. Assists with the planning and execution of forums, special events and presentations to reach and educate the community. Supports reporting and paperwork to support funding and grant requirements, including tracking patient metrics such as appointment no-shows and lost to follow up ratios. Tracks all outreach and navigation activities and provides monthly reports. Demonstrates an awareness of self-responsibility and accountability for own professional practice and development. Administers nursing care to patients if applicable including assessment, planning, implementing, and evaluating the patient's care plan in coordination with the physician. Provides orientation/education for center and hospital staff, students, and other hospitals and community organizations, as requested. Performs related duties as required. QUALIFICATIONS Hold a high school education or equivalent. Have at least 1 year experience working in the healthcare field, volunteering or personal experience with the disabled population. Current knowledge of Federal and State activity regulations and/or ability to understand and interpret. Be able to read and write English. May prefer bi-lingual abilities based off of individual facilities. Must have a valid, IL driver’s license. Must be punctual and time efficient, especially as it relates to following the activity calendar. Follow all Activity Policies and Procedures and Federal and State regulations. Become knowledgeable of each individual resident’s background, cultural, life history, disease, and medical care needs in detail to ensure an appropriate person-centered activity plan. Activities shall be daily and shall reflect the schedules, choices, and rights of the residents. Be able to understand communication needs of residents, such as language services, communication boards, etc. Per direction of the Activity Director, conduct, oversee, and complete initial and all on-going assessments, including but not limited to social, medical, cognitive, communication abilities, spiritual needs, physical, neuro-psychological, behavioral, amongst others as it relates to individual activity/leisure. Document and complete all required documentation per Activity Director. Maintain the following records: Resident Daily Participation, Resident Progress notes, Leisure Interest Survey, and Activity Planning sheets to assist in scheduling and meeting resident needs. Assist to update activity boards through direction of the Activity Director, with a monthly theme that is creative, neat, and clean. Facilitate community involvement, such as inviting the community to events and programs. Assist in resident outings, which can include but not limited to shopping centers, movies, restaurants, community events, etc. that may also entail driving residents to and from the resident outing. Follow Activity staff schedules with a team approach and continuity of care. Work as team with staff and volunteers. Must maintain structure of activity schedule and conduct all activities on-time; May not cancel an activity without permission of Activity Director. Must maintain equipment and supplies in good clean order. Must notify Activity Director within 48 hours if any new materials are needed for scheduled activities and when repairs are needed. Must update calendars and boards daily. Perform other duties as assigned.
