Asure Wound Solutions is seeking a highly skilled and dedicated Clinical Specialist, Wound Care Supplies to join our team. This is a remote position based in the Chicago area (you must live in this area) and requires experience working in nursing homes or long-term care facilities as well as a strong background in wound care. The ideal candidate must be comfortable driving distances up to 2 hours away. Ability to demonstrate a commitment to patient-focused care. Ability to prioritize independently and respond to multiple simultaneous requests. Ability to work in a fast-paced environment with established time constraints and in difficult situations. May be exposed to hazardous materials and life-threatening diseases. This position needs to recognize needs and behaviors of specific age groups and cultures of patients treated. Salary adjustments are determined by program eligibility, advancement level, and organizational guidelines. 7p-7a Conduct comprehensive clinical assessments to evaluate patients' physical, mental, and emotional health. Develop and implement individualized care plans based on assessment findings and patient needs. Coordinate multidisciplinary care by facilitating collaboration among healthcare professionals, including physicians, nurses, social workers, and therapists. Monitor patient progress, regularly review, and adjust care plans based on patient responses and outcomes. Communicate effectively with patients, families, and the healthcare team to ensure everyone is informed and involved in the care process. Document patient information accurately, including assessments, care plans, interventions, and outcomes in the patient's medical record.
