Job Details: This is a full time opportunity for experienced L&D Registered Nurses with minimally two years of L&D experience. Unit shift hours are 6:00 PM- 6:30 AM. Weekends, holidays and mandatory on call shifts are required. NRP certification within 90 days of employment required. Provides professional nursing care to patients and families in an acute clinical care environment, guided by the SMHCS nursing professional practice model and in accordance with the ANA's Standards of Professional Practice. Salary Details: Hourly rate is determined by years of worked experience as an RN, education level, and internal equity within organization. Minimum rate starting at $34.10, max of salary range is $52.80. In addition to hourly rate, if candidate holds an approved certification position qualifies for certification pay of $1.50. Department Medical Management Services Job Summary Manages the health status of the member population under the direction of the plan Medical Director. The Clinical Care Facilitator will manage the medical care throughout the continuum of care. Required Qualifications - Require a minimum of three (3) years previous healthcare experience, preferably in population care management. - Require an current/active State of Florida Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) licensure. Preferred Qualifications - Prefer previous indigent patient care management experience. - Prefer Certified Case Manager (CCM) certification from the Commission for Case Manager. - Prefer previous health plan, inpatient care and outpatient physician office experience. - Prefer familiarity with Milliman USA and Interqual guidelines. - Prefer demonstrated ability to learn SMHCS related facilities/services. - Prefer interpersonal communication skills and the ability to work effectively in a team. - Prefer demonstrated ability to work independently. - Prefer demonstrated ability to interact with SMHCS Medical Staff. - Prefer proficiency in WEB Platforms, Word, Excel, Windows and CareVision. - Prefer a Bachelor's degree in related field. Hours: Monday thru Friday, 8:30am - 4:00pm.
