Job Overview The Registered Nurse (RN) assumes the responsibility for the application of nursing and clinical processes and delivery of quality patient care at AltaMed clinics under the supervision of the Nursing Supervisor, Nursing Director, or CNE and has accountability for the nursing process and the delivery of efficient patient care. RN supports patient evaluation, and carries out related nursing interventions, supports nursing patient evaluation, efficient system applications, and recording of patient care/behavior. Works collaboratively with the clinic interdisciplinary team members, medical staff, clinical operations, and the Administrative/Operational Leadership teams to ensure the achievement of patient, clinical, or program goals as appropriate. Provides clinical care and demonstrates clinical competency in the area of specialty for patient populations. Application of and knowledge of regulatory requirements in the delegation of tasks and in documentation. Assist in the development and implementation of quality, cost-effective, efficient systems and protocols for standardizing processes of patient care at clinics. Within Health Services, may work as the Triage Nurse in Urgent Care, Primary Care Nurse in the Nurse Clinic, or the Charge Nurse on the floor at each clinic. Nursing job duties are to comply with the Nurse Practice Act, Title 22, Joint Commission (JC), CMS 42 CFR Part 460 regulatory requirements as appropriate. The plan for the provision of nursing care to patients aligns with the AltaMed Code of Conduct, professional and educational standards, and requirements as needed. Minimum Requirements Accredited school of nursing graduate with a current California State Board of Nursing Registered Nurse License required. Minimum of 1 year of experience as a Registered Nurse preferred Experience in an ambulatory care environment, i.e., clinic or Urgent Care setting, preferred. Bilingual English/Spanish, preferred. Bilingual in other languages or experience in American Sign Language is highly desirable. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines. RN Supervisor (Registered Nurse) is a licensed professional nurse directly under the supervision of the Director of Nursing Services. The RN Supervisor (Registered Nurse) is to assist the planning, organizing, developing and directing of nursing service to assure that the highest degree of quality resident care can be maintained at all times. RN Supervisor (Registered Nurse) QUALIFICATIONS • Graduate of an accredited school of nursing. • Current nursing licensure in the state in which practicing. • At least one year of experience in supervision. • Strong qualities of leadership, communication and organizational skills. • Ability to communicate effectively in English through both speech and writing. • Possess mental, emotional and physical health. RN Supervisor (Registered Nurse) GENERAL DUTIES AND RESPONSIBILITIES: GENERAL • Assure that residents have a clean, safe, orderly and comfortable environment. • Initiate resident Plans of Care on admission. • Assist in implementing recommendations from consultants and the Quality Assurance team as directed by the Director of Nursing Services. • Assist in developing, reviewing, revising an updating resident Plans of Care for individual residents. • Ensure that notification is given to the resident’s attending/alternate physician, as well as the resident’s legal guardian/representative, when the resident becomes critically ill, injured, etc., or has a significant change in condition as outlined within our established policies. • Review complaints and grievances form families and notify the Director of Nursing Services as necessary. • Evaluate emergencies and assist in implementing emergency measures. • Assure that the unit area is maintained in a clean and safe manner for resident comfort and convenience, by ensuring that necessary equipment and supplies are maintained to perform nursing care. • Assist in notification of physician of any resident incidents/unusual occurrences. Ensure that the Accident/Incident Report forms are filled out and completed on all such occurrences and such information charted in the resident’s medical record as outlined in our established policies and procedures. • Assist the Charge Nurse on investigation of alleged abuse or unusual occurrence as outlined within our established policies and procedures. • Start and monitor intravenous medication as ordered. • Assist the Charge Nurse and other nursing personnel in performing nursing procedures as necessary. • Assist the Charge Nurse in admitting, transferring and discharging residents. • Assist in correcting findings and deficiencies from the Department of Health Services, Consultants, and the Quality Assurance Team. RN SUPERVISOR – Cont’d. Job Descriptions Manual • Assist in completing restraint assessments, pain assessments, oral assessments, psychotropic assessments, bowel and bladder assessments and I & O weekly evaluations. • Assure that all nurse’s notes are charted in an informative and descriptive manner that reflects the are provided as well as the resident’s response to the care. • Attend and participate in staff meetings and in-service classes. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. SUPERVISION • Make resident rounds to ensure appropriate care is being rendered, identifying and making corrections as necessary. • Meet with nursing personnel to assist in identifying and correcting problem areas and/or the improvement of resident care. • Assist in making rounds with the attending physician as necessary. • Assist the Charge Nurse in revision and distribution of assignments in case of nursing shortage. • Assist in the supervision and direction of nursing personnel. • Participate in disciplinary action and evaluation of nursing personnel. CONSUMER SERVICE • Presents professional image to consumers through dress, behavior and speech. • Adheres to Company standards for resolving consumer concerns. • Ensures that all resident rights are protected.

Job Overview The Medical Assistant (MA) II functions as an AltaMed ambassador at the assigned clinic, PSC, or Patient Registry team. The MA is welcoming and attentive to the needs of all patients and creates a positive patient experience by managing expectations while demonstrating compassion and respect. The MA assists in the provision of direct patient care by performing routine patient care procedures and technical/clerical support services in the clinical setting. The MA builds relationships with patients, listening to them and gaining their trust. The MA demonstrates compassion and respect and will be proactive in managing patients’ expectations. In addition, the Medical Assistant documents all specialties in the Electronic Health Record (EHR) and assists with administrative responsibilities that include providing feedback on Back-Office processes to the Nursing Supervisor/Manager. Lastly, the MA assists in achieving department goals and objectives by providing support and guidance to the Medical Assistant team as a mentor and trainer. If working at the Patient Service Center, the MA provides other duties over the telephone. Minimum Requirements A Medical Assistant certificate, along with a high school diploma or equivalent required. A minimum of 1 year of experience as a back office Medical Assistant preferred in the outpatient setting, medical practice, or clinic setting. Phlebotomy certified preferred Nationally recognized Medical Assistant certification, preferred. Bilingual English/Spanish/Mandarin/Cantonese, depending on location, strongly preferred. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.
