Job Description Job Summary: The Charge Nurse works collaboratively with the manager and director to provide clinical supervision of a team of licensed and unlicensed staff during an assigned shift on a designated unit/department. Serves in a Magnet Recognized large tertiary academic medical center with level one trauma designation. Functions in a skills mix environment and utilizes discretion and independent judgment to coordinate daily operations for designated shift based on the needs of the patient population served, established professional practice model (Duffy's Quality-Caring Model), applicable regulatory standards and requirements, and approved clinical practice standards, policies and procedures. Makes assignments based on staff competency, patient acuity, staffing ratios and productivity standards. Assigns and re-assigns staff and workloads to provide uninterrupted, cost effective patient care and patient flow with optimum productivity. Flexes staff up or down as appropriate. Provides direct patient care and takes full or partial patient care list when needed, e.g., lunch relief coverage, emergency situations, high census, patient flow. Promotes the empowerment of the nursing profession and excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Facilitates a positive environment and provides hands-on guidance to engage staff in adopting a care delivery model of accountability, empowerment, and excellence in practice. Contributes to a work environment of caring and cooperation among a culturally diverse workforce and patient population. Performs other duties as needed. Education and Experience: Bachelor of Science Degree in nursing required if hired after October 1, 2020. If hired prior to October 1, 2020, Bachelor's degree may be in progress, or, an Associate's Degree in nursing with a Bachelor's degree in another field and a Master's Degree in nursing required. If hired prior to October 1, 2020, Master's Degree may be in progress. Minimum of three years nursing experience in area of specialty in a facility with an equivalent patient acuity level required. Minimum of one year of prior management or supervisory experience preferred. Knowledge and Skills: Substantial knowledge of state and federal healthcare regulations and accreditation, finance, performance improvement, quality, patient safety, evidenced-based clinical practice standards, medical staff structure, legal aspects of care, and nursing scope of practice required. Overview The DOU RN is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the therapeutic environments, treatment and modalities and interdisciplinary collaboration. Assist in tracking progress for improvement efforts related to Hospital value-based incentives (P4P, VBI, VBP, etc.), network-wide CMS star ratings, and specific performance metrics. Coordinate activities to foster good working relationships with contracted hospitals, skilled nursing facilities, hospice agencies, providers, and other external partners to encourage their participation in quality improvement groups. Help organize forums for shared learning and best practice exchange to improve patient care outcomes. Apply knowledge of clinical processes or hospital operations to support the development and implementation of specific activities (based on current results and best practices) intended to improve CMS Star Ratings, HEDIS, CAHPS, HOS, P4P, and other targeted performance measures. Gather and compile information on evidence-based practices, national and regional benchmarks, and industry standards, and help integrate those findings into current improvement initiatives. Track and monitor improvement activities by performing ongoing analysis of measured outcomes and reporting findings. Compile and summarize performance outcomes data and draft initial feedback for review by senior staff before distribution to external partners regarding improvement progress. Utilize performance improvement methodologies (i.e., Plan Do Study Act (PDSA)) to support the implementation and tracking of rapid-cycle improvement projects. Participate in outreach initiatives, and document potential barriers, experienced challenges, and activities taken to resolve issues and improve outcomes. Assist in the maintenance and organization of annual quality improvement program documents, compliance audits, policies and procedures, and related evaluations. Support the development and delivery of materials for quality improvement training sessions and problem-solving tool workshops for QI teams and stakeholders. Lead intradepartmental quality improvement teams, programs, projects, and initiatives as assigned. Continuously seek to enhance delivery and efficiency of external-facing quality improvement initiatives via technological advancement and internal-facing process improvement. Translate data into meaningful information, draw conclusions, and relate findings to industry standards. Provide recommendations to departmental stakeholders and Quality Improvement Leadership Team regarding performance gaps, program implementation, and performance outcomes in order to maintain an effective Quality Improvement program. Perform any other duties as required to ensure Health Plan operations and department business needs are successful.
