We are a premier skilled nursing and rehabilitation facility dedicated to providing exceptional care in a supportive and collaborative environment. Licensed Practical Nurse (LPN) Responsibilities: Provide direct nursing care to residents under the supervision of an RN. Administer medications and treatments as prescribed. Monitor and document resident conditions and report changes. Perform wound care and other clinical procedures. Supervise CNAs and support staff. Ensure compliance with care plans and facility policies. Maintain accurate and timely clinical documentation. Licensed Practical Nurse (LPN) Requirements: Active New York State LPN License.
