Looking for an experienced MDS RN or LPN with at least 1 year or more MDS experience (required) and preferably that experience includes some time during the last 2 years. The MDS Coordinator's responsibilities include: Provide accurate documentation of all services in order to assure accurate billing and reimbursement from all payor sources. Coordinate the development of a written care plan and assessment for each resident and review and revise as appropriate. Provide education to floor staff how to complete accurate documentation Requirements: Must be a state-licensed LPN or RN Must have 1 - 2 years MDS and Point Click Care experience Preferably have experience with PDPM Concord Village Rehabilitation and Wellness is a 72 bed facility with all private rooms located northeast of Cleveland, just off I-90 in Concord TWP, OH 44077 and we are proud to be a 5-star CMS rated facility. A Brief Overview This position is responsible for performing basic Medical Assistant duties. What You Will Do Facilitates unit operations by managing patient flow. Assist with front desk duties specific to department, including scheduling patients, collecting and verifying patient payor information and other patient documentation. Additional Responsibilities Performs other duties as assigned. Complies with all policies and standards. For specific duties and responsibilities, refer to documentation provided by the department during orientation. Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients. Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
