Overview Kelemen 10 is a medical-surgical unit that cares for patients who have various needs and disease processes. Both North and South 10 have Telemetry capabilities for our patients. Each side houses 36 patients to a maximum of 72 patients. South 10 has a 12-bed Intermediate Care Unit embedded within the 36-bed unit. INCU has critically care-trained RNs who support the patient's needs and care for ventilated patients. South 10 also has 2 negative pressure isolation rooms with an anti-room that is used when needed for specific isolation needs. We do multidisciplinary rounds Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. We have a pharmacist who has an office on the unit to assist the RNs when needed to help avoid delay of care to any patient. Education Requirements Graduate of NLN accredited School of Nursing. The Licensed Practical Nurse's primary role is to provide nursing care for patients who are experiencing common, well-defined health problems. Provides for the safe delivery of medications in accordance with the facility policies and physician orders; administers medications utilizing established safety procedures / standards. Follows up on patient care responsibilities; communicates incomplete assignments and patient needs with other disciplines to assure continuity of care; transcribes orders accurately and performs chart audits of transcription accurately and according to facility guidelines. Guided by our core values of Passion, Respect, and Excellence , the Wound Care Nurse provides skilled clinical care, assessment, and treatment of wounds while supporting interdisciplinary care planning and ensuring compliance with clinical standards. Responsibilities for Wound Care Nurse: Provide resident care in accordance with acceptable nursing standards, physician orders, and established care plans. Demonstrate sound clinical judgment in assessing, treating, and monitoring wounds. Provide wound care treatments based on resident needs and best clinical practices. Deliver respectful, compassionate, and person-centered care to residents, families, and caregivers. Provide clinical guidance and direction to members of the nursing and care team. Maintain accurate and timely documentation of wound assessments, treatments, and outcomes. Ensure resident confidentiality is maintained at all times in accordance with HIPAA and facility policies. Collaborate with interdisciplinary teams to support healing, prevention strategies, and continuity of care. Qualifications for Wound Care Nurse: Must possess a current, unencumbered, active LPN or RN license in the state of PA . Wound Care Certification strongly preferred but not required. Valid BLS Certification (CPR) required. Minimum of 2 years of prior experience as a Wound Care Nurse in a nursing home or long-term care setting required. Strong clinical assessment and critical thinking skills. Ability to work effectively in a fast-paced healthcare environment.
