Hospice Registered Nurse Clinical Director

JOB DESCRIPTION SUMMARY The Clinical Director is responsible for the overall direction of hospice clinical services according to the policies and procedures of Continuum Hospice. The Clinical Director supervises the care and services provided by the Interdisciplinary Group and delegates nursing services according to established practice standards. The Clinical Director also establishes, implements and evaluates goals and objectives for hospice services that meet and promote the standards of quality and contribute to the total organization and philosophy. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1. The Clinical Director designates an alternate, in writing, as responsible for fulfilling the duties of the DCS . 2. Supervises all direct and indirect patient services provided by clinical organization personnel. 3. Provides guidance and counseling to coordinators and Clinical Managers to assist them in continually improving all aspects of hospice care services, provided through organization personnel. 4. Assists Clinical Managers in managing clinical teams and planning. 5. Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated. 6. Assists the Executive Director/Administrator in the preparation and administration of the organization's budget. 7. Interprets operational indicators to detect census changes and increases or decreases in volume that could impact staffing levels, revenues or expenses. 8. Evaluates performance of Clinical Managers. 9. Assists Clinical Managers to develop skills and techniques in evaluating the performance of clinicians. 10. Hires, evaluates, and terminates organization personnel. 11. Conducts annual evaluations on clinicians, or more frequently if indicated. 12. Oversees the maintenance of patient clinical records, statistics, reports and records for purposes of evaluation and reporting of organization activities. 13. Assures proper maintenance of clinical records in compliance with local, state and federal laws. 14. Responsible for the maintenance of adequate and appropriate inventory supplies and equipment for the provision of patient services. 15. Develops, implements and evaluates the orientation program for new organization personnel. Responsible for orientation of new organization personnel, directly or delegated. 16. Plans and implements inservice and continuing education programs to meet education and training needs of organization personnel at least quarterly. 17. Assists with the evaluation of organization performance via performance improvement program, productivity, quarterly and annual reviews. Assures for the quality and safe delivery of hospice services provided through the Organization. 18. Assists in the development of organization goals. Develops, recommends, and administers Organization policies and procedures. 19. Assures compliance with all local, state and federal laws regarding licensure and certification of organization personnel and, maintains compliance to the CHAP Accreditation standards. 22. Stays informed about changes in the field of nursing and hospice care; shares information with appropriate organization personnel. 23. Promotes hospice referrals in the health care community. 24. In the absence of the Executive Director/Administrator, the Clinical Director becomes the acting Executive Director/Administrator and will be vested with authority to act in behalf of the Executive Director/Administrator. 25. Other duties as assigned by the Executive Director/Administrator. POSITION QUALIFICATIONS 1. Graduate of an accredited school of nursing 2. Registered Nurse, with a License in good standing to practice in the state of Ohio. 3. Either a Bachelor’s degree in nursing with three (3) years of management experience within the last five (5) years in a hospice or related health care organization, at least one (1) year of which was in a supervisory or administrative capacity, OR A Registered Nurse with four (4) years’ experience within the last five (5) years in a hospice, home health agency, primary care clinic or health facility, at least one (1) year of which was in a supervisory or administrative capacity. 4. Once an offer of employment is made, it is contingent upon satisfactory references, as requested, and criminal background checks by regulation. 5. Demonstrated ability to supervise and direct professional and administrative personnel. 6. Ability to market and deal tactfully with customers and the community. 7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order. 8. Has excellent observation, verbal and written communication skills. 9. Possesses and maintains CPR certification (may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 10. Certified Hospice and Palliative Nurse preferred 11. Knowledge of business management, governmental regulations and CHAP standards is preferred.

From bariatric surgeries like gastric bypass and gastric sleeve to hysterectomies and other general surgical procedures, this unit is at the forefront of surgical recovery. Patients also come to the unit with complex medical conditions such as small bowel obstructions (SBO), gynecological oncology issues, and those requiring advanced therapies like NG tubes, TPN, and electrolyte replacement. The care team is highly skilled in managing wound vacs, surgical drains, and ostomies, with the added capability of remote telemetry monitoring. The unit fosters a supportive and flexible work environment, offering 12-hour shifts with self-scheduling. Nurses work within a 6-week schedule that incorporates weekend and holiday rotations, ensuring a healthy balance between work and personal life. New nurses are welcomed with a 6-week competency-based orientation guided by an experienced preceptor. Job Description Ventricular Assist Device (VAD) Patient Coordinator is a clinical expert registered nurse who applies clinical expertise in evidence-based practice. Applies concepts derived from management and communication theory, group dynamics and clinical judgment to implement the VAD program. Integrates care with multidisciplinary team (within the hospital, practice and in the community) to facilitate VAD patient care from pre implant thru discharge of care. Clinical Expert/Consultant : Demonstrates or supports implementations of nursing process for VAD/HF patients. Recognized advisor sought out for clinical expertise related to VAD/HF. Responsibilities Educator : Evaluates competency of clinical staff (inside and outside of the hospital), patients, family members, physicians, and community health resources related to the care of patients with VAD's. Researcher : Functions as Study Coordinator or Co-Investigator for national investigational studies related to VAD and HF conducted at TCH. Utilizes and communicates applicable research to improve practice. Case Manager : Functions as case manager for all VAD patients from time of referral to implant through discharge with ongoing case management thereafter. VAD Patient Care Manager : Manages equipment, supplies, policies and procedures related to the VAD Program as assigned. Customer Satisfaction : Actively promotes improved relationships between patients, nursing staff, internal and external customers resulting in improved customer satisfaction. Maintains a data base to provide reports to medical teams, TCH, and others as requested. Actively participate in local and national meetings to exchange knowledge and information in the area of cardiac surgery including valve replacement technique Collect data required by the hospital, the FDA, and other regulatory bodies related to implanted devices and submit to the appropriate agency(ies). Coordinate list for implantation and changes of status, including forms completion and computer entry. Coordinate readmissions for patients having difficulties that require inpatient management. Develop and integrate new matrices and their implementation into existing practices for the PV Program and develop Policies and Procedure with the medical director oversight. Acute care experience (critical care/transplantation/VAD/HF experience) is preferred. REQUIRED SKILLS AND KNOWLEDGE: Knowledge of computers and regulatory agency reporting requirements. Previous experience in developing education materials and providing education to various populations, both patient and professional. Excellent communication skills. Demonstrates and models compassionate behavior to patients, staff, and the community. LICENSES & CERTIFICATIONS: RN licensed in the State of Ohio required.
