Job Description The Heart Transplant Nurse Coordinator for the Advanced Heart Failure Comprehensive Care Center provides direct care to patients with multiple, often complicated acute and chronic medical problems. The Nurse Coordinator is the primary point person for the program in the post heart transplant phase of care and works closely with the multidisciplinary team and multiple UCSF departments. Job Description UCSF nurse practitioners (NPs) are advanced practice providers (APPs) who possess the education, national board certification, California licensure, knowledge, clinical skills, and experience necessary to provide evidence-based patient care in their area of clinical practice.
