About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the Connecticut shoreline (Groton-New Haven) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow. What you'll do Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: what matters most to the resident, medication review and deprescribing, cognition (including the Mini-Cog), mood, sleep, nutrition, mobility, fall risk, daily function and advance care planning Evaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriate Identify residents who would benefit from follow-up care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry or therapy, and coordinate with their primary care provider Document every encounter accurately and completely, to Medicare standards, in our EHR Represent Total Life professionally with residents, families and community staff Travel to assigned senior living communities in the Connecticut shoreline (Groton-New Haven) area Requirements Active, unrestricted Connecticut APRN license as a nurse practitioner from the Connecticut Department of Public Health, plus an active Connecticut or multistate RN license (a multistate RN compact license does not cover APRN practice) Current national NP board certification Active DEA registration and a Connecticut Controlled Substance Practitioner Registration from the Department of Consumer Protection Either independent practice eligibility in Connecticut (licensed 3+ years with 2,000+ hours of physician collaboration, with notice filed with the Department of Public Health) or the ability to practice under a written collaborative agreement with a physician, as required in Connecticut Clinical experience caring for adults 65+ Experience with Medicare documentation and E/M coding Your own malpractice insurance (minimum $1M per occurrence, $3M aggregate) that covers in-person care, not just telehealth, or the ability to obtain it before your first site day Comfort presenting to groups and speaking with older adults and their families A valid driver's license and reliable transportation Ability to complete each community's onboarding requirements, which may include a background check, TB screening and immunizations Weekday availability for on-site days in the Connecticut shoreline (Groton-New Haven) area, with hours confirmed in advance Nice to have Experience with comprehensive geriatric assessment Familiarity with cognitive screening and staging tools (Mini-Cog, MoCA, FAST); MoCA certification is a plus Experience in senior living, long-term care, SNF, hospice, palliative or house-call settings Experience prescribing GLP-1s for weight management Telehealth experience Licenses in more than one state Adult-Gerontology NP certification (AGNP/AGPCNP), or Family NP with a geriatric focus or training What we offer Mileage reimbursement for site days Training from our CMO in geriatric assessment, cognitive tools (Mini-Cog, MoCA, FAST) and our documentation Presentation materials, resident sign-ups and scheduling handled by our team Flexible contractor schedule, with hours that grow as your calendar fills The chance to help build a program for a population the longevity field has overlooked Total Life welcomes applicants of all backgrounds and is committed to equal opportunity. Clinical Care * The Physician Assistant (PA) or the Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team. The NP/PA, whether the practice setting is the inpatient, ambulatory, or outpatient settings, provides optimal patient care, from diagnostic to prognostic matters. This individual works in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols and clinical pathways and may provide autonomous team based care. * Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic results, formulates and manages patient treatment plans. The NP/PA may perform procedures, prescribe treatment and medications in accordance with institutional and state guidelines, as well as, when necessary for scope within specialty or professional competency standards. In some clinical specialties, the NP/PA may have autonomous practice managing certain patient populations. * The NP/PA may also function as first or second assistant on operative cases (as long as within position scope and job educational prerequisites are in place), using fine motor skills and manual dexterity. * Other clinical job duties may be applied in accordance with departmental and operational goals, training related to profession and practice necessitated guidelines and responsibilities, inclusive but not limited to responding to emergency situations within the YNHHS parameters 2. Patient Education & Psychosocial Support * The NP/PA will educate patients/families on care and recommended or available resources. In collaboration with the supervising/collaborating MD, this may be inclusive of admitting instructions, treatment plan, actual and potential side effects of treatments/medications, and discharge plan. * The NP/PA, as part of a multidisciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner. 3. Information Management * The NP/PA ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure. 4. Quality Management * The NP/PA is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations. 5. The Admitting Nurse serves as a member of the Interdisciplinary Team (IDT) and is primarily responsible for admission and resumption of care visits of patients for home care and hospice services, development of the patient plan of care, coordination with the patient's provider and other disciplines for the provision of services, and safe delivery of patient care. Provides a complete physical assessment and history of current and previous illnesses to support eligibility for hospice services. 2. Assesses/reassesses patients and establishes plans of care that are patient-specific. 3. Collaborates with providers, other disciplines, and communicates all changes promptly. 4. Educates patients/caregivers regarding hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary. 5. Completes all documentation within twenty-four (24) hours. Must include changes in condition, follow-up with the provider or other disciplines, with supporting documentation. 6. Oversight of LPNs and Home Health Aides. 7. Reviews and updates the POC per regulatory standards. 8. Schedules patients independently according to the individual plan of care. 9. Thorough and accurate CMS data collection tool documentation. 10. Builds and nurtures relationships with referral sources, physicians, and facility staff to enhance visibility and trust in hospice services. 11. May be asked to participate in Interdisciplinary Team (IDT) case management conferences and documents per guidelines. 12. Attends all staff meetings, in-services, and other required meetings. A valid driver's license and proof of car insurance are required when using a personal vehicle for company business. Access to a dependable vehicle in order to travel to multiple stops a day.
