Schedule Monday - Friday 7:45 am - 4:15 pm No weekends/holidays General Summary Works in collaboration with the primary care providers and all members of the health care team. Primary responsibilities include identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and execution of the guided processes to complete the AWV. Assesses healthcare needs that require additional follow up by primary care provider and assists patients in scheduling appropriate follow-up. This position requires expertise in the nursing process, using critical thinking skills, plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations as a member of the clinical team. Must be an innovative thinker and open to changing the way we deliver care. Duties may include quality monitoring, patient assessments, direct patient care, and patient/staff education. Provides safe, therapeutic, efficient quality care and services to all patients served.
