Develops strategies to improve patient/family, and physician satisfaction. Establishes standards of care for professional nursing practice that staff are held accountable to. Monitors critical processes and outcomes of care through audits, analysis of data, and complaints or incident reports. Exercises discretion and independent judgment with respect to matters of significance, evaluating and comparing possible courses-of-action, and making decisions/recommendations after considering the various possibilities. Conducts daily rounds with physicians and other staff and actively communicates, as needed, to coordinate appropriate care for patients and families. Directs/monitors personnel in the performance of patient care activities in order to ensure adequate patient care and quality of work. Investigates and resolves patient care and operational issues, as needed. Initiates and coordinates individual and team conferences with health team members, patients, and/or families to initiate and revise care plans for optimal, individualized patient care as per care setting. Assesses the personnel requirements of the unit on a daily basis and requests additional personnel when determines they are necessary. Authorizes and assigns overtime to personnel when independently determines this to be appropriate. Completes monthly unit staff schedule on time, accurately, and in collaboration with staff members and management with minimal incidents of absence or unbalanced/short staffing events. Facilitates throughput through early discharge of patients, pull-ahead beds, and timely admission/transfer of patients and forecasting admission. Works with facility services to assure that the department and equipment is maintained to be safe and operational. Prevents complications of care including nosocomial infections. Conducts emergency preparedness reviews. Performs other job-related duties as assigned. We are seseking a DYNAMIC Director of Nursing for a 99 bed facility in the Stockton area. The ideal candidate will hold a current RN California License, have an excess of two years’ experience as a Director of Nursing or nursing management, be savvy to working within an electric record, have familiarity with MDS, possess knowledge of the regulatory climate of California. Assures nursing staff properly charges out ancillaries used. Helps the Administrators prepare staff for inspection surveys, instructing staff on matters of conduct and disclosure, being interviewed by inspectors, immediate corrections of problems noted by surveyors, etc. Reviews and reinforces important standards previously cited. Participates in the preparation of the Plan of Correction response to an inspection survey, and implements any follow-up QA required for any nursing allegations. Communicates directly with residents, families, medical staff, nursing staff, interdisciplinary team members, and Department Heads to coordinate care and services, promote participation care plans, and maintain a high quality of care and life for residents. Promotes customer service and hospitality and responds to and adequately resolves complaints or concerns from residents or families about nursing services. Monitors facility incidents and complaints daily to identify those defined as unusual occurrences by State policy and promptly reports such occurrences to Administrator/Executive Director for appropriate action. Monitors complaint reports daily for allegations of potential abuse or neglect, or the loss or misappropriation of resident property, and participates in these investigations. Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness and control worker’s compensation costs. Assures staff is trained in fire and disaster and other emergency procedures, and evaluates performance during drills. Interacts courteously with residents, family members, employees, visitors, vendors, business associates, and representatives of government agencies. Oversight of finances for the Clinical department is a must, controlling expenses and guiding designees to do so will be the expectation based on spend down or cost control budgets Acts in an administrative capacity in the absence of the Administrator. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. PEDIATRIC HOSPICE RN CASE MANAGER JOB DESCRIPTION Overview The Hospice Pediatric RN Case Manager will work with our Pediatric team. Primarily responsible for providing professional nursing care to assigned patients. With input from the IDT (interdisciplinary team), patient and family, assesses, develops, implements and evaluates the plan of care. Ensures appropriate communication and plan of care coordination to all internal/external customers All successful candidates must be fully vaccinated, subject to certain limited exceptions required by applicable law. DEGREE OF TRAVEL: • Travel required for patient visits. • Occasional travel for educational purposes. Qualifications: • Must have Associates Degree, Bachelor Degree preferred. • Must have one year of acute or hospice pediatric experience • At least 12 months experience with 1-2 years preferred in acute care. • Current CA RN license. • Current CA driver’s license with proof of insurance. • Current CPR/BLS • Prefer certification as hospice nurse. • Pediatric experience preferred. • Proof of recent TB test result • Bilingual preferred. • Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
