Patient Navigator (RN) - Cancer Care

Seeking Patient Navigator positionsupporting the Head/Neck/Thoracic Medical Oncology Team, which specializes in caring for patients with diagnoses including lung cancer, mesothelioma, esophageal cancer, thyroid and salivary gland tumors, adrenocortical carcinomas, peripheral nerve sheath tumors, cutaneous malignancies, and sarcomas. This role will begin with developing a strong understanding and confidence in the RN triage function, with the expectation that the nurse will progressively expand into patient navigation responsibilities as they grow within the practice. The ideal candidate will have three or more years of oncology experienceand demonstrate foundational leadership abilities that can be applied in this setting. This individual will serve as a change champion, supporting team initiatives and contributing meaningful value to the ongoing growth, development, and forward progression of the practice. Summary The Registered Nurse – Patient Navigator – MHMG MMCP role guides patients through the care continuum, establishing contact with recurrent and newly diagnosed cancer patients and/or family members, or caregivers. The Patient Navigator will understand the clinical pathways established by the Oncology Program and will direct the patients to health care services within the organization, at outside facilities, and within the community for timely diagnosis, treatment and survivorship. This is a 24 hour/week position on the night shift (7p-7a). This position will work in the Emergency Dept Behavioral Health Suite. The Work: Plans, implements, and evaluates patient care plans based on patient assessment to optimize outcomes and maximize available resources. Monitors, records, and communicates patient condition as appropriate. Collaborates as needed across disciplines to coordinate patient care, including patient transfer, admissions, discharge, referral and spiritual/psychosocial support needs. Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances. Effectively delegates and communicates patient care needs to ancillary personnel. May precept student nurses and new hires.
