MDS Coordinator – RN or LPN The Springs at Rochester Hills is seeking an experienced and detail-oriented MDS Coordinator (RN or LPN) to join our skilled nursing team. This is an excellent opportunity for a knowledgeable nurse who understands the MDS process and is committed to accurate assessments, regulatory compliance, and exceptional resident care. Position Summary The MDS Coordinator is responsible for coordinating and completing the Resident Assessment Instrument (RAI) and MDS process in accordance with federal and state regulations. This position works closely with nursing, therapy, dietary, social services, and other members of the interdisciplinary team to ensure assessments and care plans accurately reflect each resident’s needs.
