Job Summary The Community Health Nurse is responsible for providing skilled professional nursing care to the patients assigned to him/her in a comprehensive manner promoting quality, continuity and safety. Completes, maintains and submits accurate, legible and relevant nursing paperwork regarding the patient's condition and all care and treatment given. Monitors patient expectations and satisfaction. Accepts assignments and performs duties within the limits of preparation and experience, seeking and obtaining sufficient information necessary to function effectively in the home health care setting. Participates in appropriate agency meetings and the quality improvement and utilization review activities of the organization. Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education, in-service programs, professional conferences and community and agency.
