Position Overview: This role is based at a healthcare facility in the Bronx, supporting admissions, care planning, and overall nursing department operations while ensuring high-quality resident care. Admissions experience is a must! Duties & Responsibilities: Conduct admission assessments for new residents and hospital returns; initiate care plans. Review PRI documentation and coordinate all aspects of the admission process with the interdisciplinary team. Assist in the development, implementation, and oversight of nursing policies, procedures, and department operations in collaboration with the Director of Nursing (DON).
