Job Summary: Independently manages the care of low-risk women during antepartum, intrapartum and postpartum periods. Co-manages higher risk patients when determined appropriate by CNM and MD. Provides standard maternity care and counseling to women, adhering to the standards of the American College of Nurse Midwives, procedures and instructions regulated by the State Nurse Practice Act and The Permanente Medical Group. Essential Responsibilities: ANTEPARTUM Evaluates patients general and obstetrical health status Determines pregnancy status, duration of gestation, presentation, and position of fetus, fetal heart rate, and determines whether all statistics are within normal limits. Obtains an accurate medical, obstetrical, gynecological and social history and an account of the course of the present pregnancy. Observes signs of the members general and obstetrical health status. Performs all appropriate physical examinations appropriate to CNM care needed, including the pap smear, breast examination, and pelvic examination. Records all findings of examinations, histories and significant observations on patients chart. Consults with appropriate MD regarding any abnormal findings. Orders appropriate lab and medical imaging studies as needed. Institutes preventative measures and detects abnormal conditions. Obtains consultation and assistance from ancillary services as needed. Teaches, counsels, and guides individual members regarding their individual health care. Teaches antepartum classes as needed in individual facilities. Evaluates maternal general health status and well being. INTRAPARTUM Performs medical screening exam to diagnose labor. Independently manages the care of low risk women during labor and delivery, under the supervision of a physician. Co-manages higher risk patients when determined appropriate by CNM and MD. Performs facility specific standardized procedures. Evaluates patient for appropriate pain management. Includes requesting epidural for women in labor. POSTPARTUM Provides health supervision, care, teaches family planning, and prescribes contraception to women during the postpartum period. Position Description We are looking for an outstanding MDS nurse to work closely with our facilities in NorCal- Stockton and Sacramento. This consultant will be responsible for providing expertise, leadership, modeling, and support for organization-wide initiatives. He/she will also work on the implementation and or evaluation of the facility’s Quality Measurement and Performance Improvement program to ensure quality of resident care outcomes. Qualifications · Must possess a current, active license to practice as a registered nurse in this state. · 3 years’ experience as a registered nurse, one of which includes supervisory experience, preferred. · Experience with MDS completion and process. · Resident Assessment Coordinator (RAC) Certification is preferred. · Understanding of computer technology, including Electronic Health Record (EHR) systems Duties and Responsibilities · Overseeing assigned facilities’ comprehensive resident assessment process in accordance with state and federal regulations. · Auditing completion of MDS assessments and any supporting assessments or clinical documentation. · Reviewing medical records for the presence of supporting documentation for all items coded on the MDS. Provide education as necessary. · Interpreting rules, regulations and coverage guidelines and acting as primary resource for problem solving regarding care-based reimbursement systems and quality reporting program. · Provide consultative support and training to MDS Coordinators within the assigned facilities, to coordinate and guide resident-centered care. · Assist the Interdisciplinary Team with Care Planning. · Provide additional guidance related to clinical documentation and coding as it relates to the Resident Assessment Instrument (RAI). · Provide support and guidance to assist assigned facilities to achieve and maintain 5-Star Quality Measure Rating. · Auditing and monitoring of RAI timeliness and accuracy of information in furtherance of regulatory standards. · Support resident care by identifying trends and developing processes and action plans. Must be able to perform and present Root-Cause-Analysis. · Reviews and audits data to support assigned facilities’ obligation to accurately capture diagnosis codes in accordance with PDPM guidelines. · Provide on-going education, support, and make recommendations to optimize quality of care delivery. · Attend and support assigned facilities’ Quality Assurance Performance Improvement Programs (QAPI). · Work as an advocate for MDS Coordinators in relationship to the facility leadership, and the Interdisciplinary Team (IDT). · Maintain the most current knowledge of State and Federal guidelines and regulations related to the RAI and reimbursement. · The Regional MDS Nurse will assist the facility to prepare for the surveys and should be available to assist during the survey. Duties of Hospice Registered Nurse Include: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness. Conducts home visits and assesses/evaluates patient's status. Initiates the plan of care and makes necessary revisions as patient status and needs change. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Administers medications and treatments as prescribed by the physician. Attends and participates in Interdisciplinary Group Meetings. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Supervises ancillary personnel and delegates responsibilities when required. Qualifications: Graduate of accredited school of nursing Current RN license in California Current CPR Certificate Registered nurses shall have a minimum of (a) one year of experience as a professional nurse within the last three years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and current RN license. The RN is responsible for answering calls from patients, families and customers. The RN will make nursing visits for symptom management, deaths, and admissions as needed. This position will be after hours on-call working seven days/nights on and seven nights/days off.
