Position Description Certified nursing assistant (CNA) responsible for delivering and assisting resident with daily needs, including personal care needs, preparation for activity and social programs, and routine activities in accordance with accepted standards of practice, state and federal regulations and licensing requirements. Provides basic nursing care to residents within the scope of the nursing assistant responsibilities and performs basic nursing procedures under the direction of the licensed nurse supervisor. Principal Responsibilities • Provides residents’ personal hygiene by giving bedpans, urinals, baths, back rubs, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths. • Provides for activities of daily living by assisting with serving meals, feeding residents as necessary; ambulating, turning, and positioning residents. • Assists with lifting, positioning and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc., in keeping with specific resident safety needs; Assists residents in preparation for and transporting to appointments, activities and social programs • Participate in meal service including serving, clearing, and cleaning the dining room; Assures that residents are positioned appropriately for meals; Assures that appropriate utensils are within reach of residents; Serves food trays and assists with feeding as indicated; Delivers and documents all a.m., p.m., and bedtime snacks and supplements; Provides hydration to all residents according to center policy. • Responsible for knowledge of proper procedures to provide personal care services to residents; provide requested personal care services to residents. • Prepares equipment and aids nurses during treatments and examination of residents. • Checks residents in section regularly; Answers call light promptly; Follows through with resident problems or needs by delivering care or reporting to supervisor depending upon level of ability and nature of resident need. • Maintains resident stability by checking vital signs and weight; testing urine; recording intake and output information. • Provides resident comfort by utilizing resources and materials; transporting residents; answering resident call lights and requests; reporting observations of the resident to nursing supervisor. • Documents actions by completing forms, reports, logs, and records. • Protects company’s integrity by keeping resident information confidential. • Serves and protect the center’s community by adhering to professional standards, company policies and procedures, federal, state, and local requirements. • Review care plans routinely to determine if changes in the residents’ care routines have been made. • Participates in and receives nursing report upon reporting for duty and during shift change. • Makes beds (occupied and unoccupied); Changes bed linens and keeps linens tight to avoid wrinkles from forming under residents; Handles linens according to infection control guidelines. • Keeps residents dry (changes gown, clothing, linen, etc., when it becomes wet or soiled); Assists residents with bowel and bladder functions. • Maintains cleanliness of resident units, central bathing area, treatment and supply rooms, workrooms and linen closets; cleans bedpans, urinals and commodes; washes and empties basins; sanitizes bath tub after each resident’s use; cleans floors if resident is incontinent; wipes spilled foods and liquids. • Provides post-mortem care as instructed. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Certified in the state of practice, required. • Successful completion of State Approved Nurse Aide Training and Competency Evaluation Program; Name must appear on State Registry as Nurse Aide in good standing. • Prior experience as Nurse Aide in a skilled nursing facility preferred but not required. • Current BLS/CPR certification (All SNFs; ALFs as required by state). Benefits Offered: Healthcare Dental Vision PTO 401K Your Job Summary The MDS Coordinator will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, may coordinate information systems operations and education for the clinical department. Principal Responsibilities • Works in collaboration with the Interdisciplinary Team to assess the needs of the resident; Provides interdisciplinary schedule for MDS assessments and care plan reviews as required by governing agencies. • Ensures that the Interdisciplinary team makes decisions for either completing or not completing additional MDS, assessments based on clinical criteria as identified in the most recent version of the RAI User’s Manual. • Assist with coordination and management of the daily stand up meeting, to include review of resident care and the setting of the assessment reference date(s). • Complies with federal and state regulations regarding completion and coordination of the RAI process. • Monitors MDS and care plan documentation for all residents; ensures documentation is present in the medical record to support MDS coding. • Maintains current MDS status of assigned residents according to state and federal guidelines. • Maintains the frequent and accurate data entry of resident information into appropriate computerized MDS programs. • Completes accurate coding of the MDS with information obtained via medical record review as well as observation and interview with facility staff, resident and family members. • Attends interdisciplinary team meeting, quality assurance and other meeting in order to gather information, communicate changes, and maintain and update records. • Assists DON or designee with identification of a significant change, physician orders and verbal reports to assure that the MDS and care plan are reflective of those changes. • Prepares scheduling, notice of resident care planning conferences, and assists DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party. • Continually updating knowledge base related to data entry and computer technology. • Completes electronic submission of required documentation to the state database and other entities per company policy. • Corrects and ensures completion of final MDS and submits resident assessment data to the appropriate State and Federal government agencies. • Assigns, assists, and instructs staff in the RAI Process, PPS Medicare, Medicaid (Case Mix as required) and clinical computer system in relation to these processes. • Maintains confidentiality of necessary information. • Other duties, responsibilities and activities may change or assigned at any time with or without notice. Qualifications • Graduate of an approved Registered Nurse program and licensed in the state of practice, required. • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred. • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required. • Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set. • Knowledge of the care planning process. • Experience with MDS 3.0, required.
