Description Job Title: Clinical Nurse-Med-Surge-Tele-KCC5N-Mount Sinai Hospital-FT/Days The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. Responsibilities Patient Care Delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. Manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates allocation of material resources effectively. Patient Experience Role models the tenets of Mount Sinai Relationship Centered Care (MSHRCC) through facilitation of the following actions including, but not limited to, Admission Welcome, Bedside shift Report, HELP (High Risk Medications, Equipment, Lines, Drains, Airway, Pain and Plan of Care), AM/PM Care, 5 Minute Sit Down, Purposeful Hourly Rounding, Medication Review, Discharge Wrap Up, and MD/RN Unit Collaboration. Considers the patients values, preferences, expressed needs and knowledge in all aspects of care. Establishes and maintains a therapeutic relationship with the patient and family. Supports staff use of evidence-based practices to increase understanding of patient's perceptions of care. Conducts Purposeful Hourly Rounding (PHR) on assigned patients addressing the 4 Ps. Quality and Safety Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements process improvement strategies based on institutional, evidence-based ensure and procedures. Engages in formal and informal peer and institutional review processes. Collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities and nursing research studies. Contributes to quality and safety practices and required compliance measures. Implements principles of high reliability to identify and avoid high risk behaviors to provide a safe environment for patients. Escalates potential safety hazards or gaps from best practice following institutional chain of command protocol. Utilizes institutional information technology by documenting nursing practice to support quality and performance improvement initiatives. Operations Performs charge nurse duties as assigned. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. What You'll Be Doing Under the general supervision of the physician and/or Director of Invasive Cardiology, the Cardiac Cath. Lab. Nurse is responsible for assessing nursing needs and initiating appropriate Intervention for Adult and Geriatric patients in Cardiac Cath. Lab, Department as required. Maintains standards of nursing care and practice of the Nursing Department. What We're Looking For Graduate from an approved school of nursing. BCLS, ACLS ,IABP Certification. Complete IV Certification course and Telemetry certification as per Medical Center policy. Knowledge of radiographic procedures and equipment desirable. A minimum of one (1) year previous acute care experience is required to qualify for sign-on bonus(*) Experience in Medical-Surgical Nursing; 1-2 Years Critical care experience required. Ability to act in a professional capacity and serve as a professional role model and function independently with minimal supervision. Current Registered Nurse License in the State of New Jersey. BLS, ACLS, IABP required. PALS certification and experience in EKG interpretation, hemodynamic monitoring. PTCA experience preferred. Full-time, day shift. 4 days a week, 10-hour shift (Mon-Fri). Job Description The Pain Management Center provides care to outpatients. The hours of operation for the Pain Management Center are Monday through Friday 7:30 AM until 4:00PM. The responsibility of the department is to ensure high quality nursing care to patients having Pain Management procedures according to American Society of Pain Management Nurses (ASPMN), American Heart Association, ASPAN, NJ State Department of Health, CDC, CMS, OSHA and TJC recommendations. The department provides peripheral nerve blocks, nerve injections, nerve ablations, tenotomy and epidural procedures. The scope of care includes independently assessing patient needs, making collaborative decisions relative to all phases of care, pre-procedure, intraprocedural, and recovery while maintaining patient safety, monitoring, and high-quality patient care. Principal Accountabilities Maintains accurate, complete health care records and reports. Administers medications to patients and monitors them for side effects and reactions. Prescribes assistive medical devices and related treatments. Monitors, reports, and records symptoms or changes in patient conditions. Assesses, implements, plans, or evaluates patient nursing care plans by working with healthcare team members. Records patient vital signs and medical information. Performs other related duties as assigned. Required QUALIFICATIONS Graduate of an accredited Nursing Program. Current state licensure as a Registered Nurse. BLS certification. Preferred Bachelor’s degree in nursing. All ASN must complete their BSN studies within five years of their start date. 1+ year of experience in Nursing or relevant area. ACLS certification.
