The Wound Ostomy Continence Nurse ensures prompt, efficient, and effective patient care and appropriate delegation of related clerical and program support activities. Responsibilities Supports the Wound, Ostomy, Continence Care Manager in the development and implementation of the WOCN Program to meet the needs of patients and community. Guides and collaborates with the nursing staff, medical staff, patient, family and other members of the health team to assess, plan, implement, evaluate, and improve the nursing needs of patients with Wounds, Ostomies, fistulas, drains or altered skin integrity including physical, emotional and spiritual needs. Assesses the nursing and medical staff’s educational needs and collaborates with the Wound, Ostomy, Continence Care Manager to develop, implement, and conduct a variety of teaching programs regarding WOCN needs. Provides resources to clinical preceptors to enhance the learning process during clinical orientation/rotation. Assesses, plans, implements, and evaluates the plan of care for continuity of care in collaboration with the medical staff, nursing staff, social worker, and other health care teams, including care after discharge. Evaluates patients’ response to treatments and initiates alternative treatment approach, when indicated. Assesses physical, emotional and social factors that may affect the patient’s rehabilitation. Functions as a consultant to the staff for patients with complex draining wounds, fistulas, pressure ulcers, and skin problems related to fecal and urinary incontinence. Enhances professional growth and development through participation in educational programs, nursing leadership meetings, hospital-wide committee (Infection Control and QI), and maintaining high level of competence in WOCN specialty. Verbalizes and/or demonstrates knowledge of clinical information and practices essential to provide age-specific care for the infants through the geriatric population. Develops and administers departmental standards of performance, policies and procedures. Assists in making purchasing recommendations for ostomy and wound care supplies. Assists in planning and implementing program, staffing, supervision and program development. Assists in developing and implementing a program of professional and community contacts. Assists in developing and implementing professional growth activities for all staff. Maintains an effective liaison with physicians and other hospital personnel. Maintains open lines of communication between Department Staff, Administration and other departments of the hospital. Acts as a resource for staff regarding the contents of applicable professional codes, SRRMC/Tenet employee handbooks, and departmental P&P and hospital policies. Maintains current competence and skill levels required for work areas. Adheres to policies and practices of applicable professional organizations. Demonstrates knowledge of criteria/accreditation standards (JCAHO, CARF, CHFA, ETC) and assists with survey preparation as assigned. Qualifications Minimum Education: Graduate of an accredited RN Program BSN and Wound, Ostomy and Continence Certification, and experience preferred Minimum Experience/Skills: Minimum of one (1) years experience in an acute clinical nursing department, ideally a medical surgical or specialty area Minimum of one (1) year experience as Wound, Ostomy, and Continence Nurse preferred Experience where leadership, integrity, initiative and effective communication skills have been demonstrated Must have knowledge of adult education theory Licenses/Certificates/Credentials: Current California RN license Current BLS certification for Healthcare Providers (AHA certified) Completion of an accredited Wound, Ostomy, Continence Training Program preferred Current Certification in Wound, Ostomy, and Continence preferred. Assesses the nursing and medical staff’s educational needs and collaborates with the Wound, Ostomy, Continence Care Manager to develop, implement, and conduct a variety of teaching programs regarding WOCN needs. Provides resources to clinical preceptors to enhance the learning process during clinical orientation/rotation. Assesses, plans, implements, and evaluates the plan of care for continuity of care in collaboration with the medical staff, nursing staff, social worker, and other health care teams, including care after discharge. Evaluates patients’ response to treatments and initiates alternative treatment approach, when indicated. Assesses physical, emotional and social factors that may affect the patient’s rehabilitation. Functions as a consultant to the staff for patients with complex draining wounds, fistulas, pressure ulcers, and skin problems related to fecal and urinary incontinence. Enhances professional growth and development through participation in educational programs, nursing leadership meetings, hospital-wide committee (Infection Control and QI), and maintaining high level of competence in WOCN specialty. Verbalizes and/or demonstrates knowledge of clinical information and practices essential to provide age-specific care for the infants through the geriatric population. Develops and administers departmental standards of performance, policies and procedures. Assists in making purchasing recommendations for ostomy and wound care supplies. Assists in planning and implementing program, staffing, supervision and program development. Assists in developing and implementing a program of professional and community contacts. Assists in developing and implementing professional growth activities for all staff. Maintains an effective liaison with physicians and other hospital personnel. Maintains open lines of communication between Department Staff, Administration and other departments of the hospital. Overview JOB DESCRIPTION – POSITION REQUIREMENTS JOB TITLE: Staff Nurse II CC/Department: 7010/Emergency Services Reports to: Administrative Director of Emergency Services JOB SUMMARY: The Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at Seton Medical Center and Nurse Practice Act. Responsibilities Clinical Practice: Nursing 1 Assesses, collects and documents health assessment data focusing on the physical, emotional, spiritual, psychosocial and educational needs of the patient. a) Interviews the patient/family/significant other to complete Triage information. b) Identifies actual and potential needs and determines priority. c) Identifies barriers to learning. After initial triage evaluation, completes and documents an assessment, focusing on abnormal systems and actual and potential problems. Performs ongoing assessment as indicated. 2 Plans nursing care, based on priority of physical and psychosocial-emotional needs. a) Develops realistic short-term goals for each problem, involving the patient, family or significant others. b) Plan demonstrates understanding of age-related needs of the patient. c) Negotiates with patient, family, significant other and health care team members in planning for education and discharge needs. 3 Implements the plan of care including prescribed procedures and treatments. Organizes and prioritizes care based on individual needs and unit activity. Assesses for changes in patient’s condition and appropriateness of the plan. Initiates interventions using appropriate clinical expertise and knowledge. As Primary Nurse, coordinates the activities of health care team members in completing diagnostic and therapeutic interventions. 4 Evaluates the effectiveness of nursing care delivered to reach the expected outcome. a) Evaluates the plan of care according to the current condition and changing needs of the patient. b) Accurately and concisely documents the patient’s status and response to the care delivered. Clinical Practice: Emergency Nursing 6 Utilizing professional nursing knowledge and skills. Appropriately assesses for: Physical and psychosocial-emotional needs. Barriers to learning. Educational/learning needs. Discharge needs. Age-related needs. Issues related to assault or abuse. 7 Analyzes patient related data, such as vital signs, past medical history, physical exam findings, laboratory values and other information, including age-specific criteria, to determine appropriate care. 8 Utilizes advanced, specialized knowledge and skills to implement interventions for all Emergency Services patients, by prioritizing needs, and using a patient centered approach. 9 Provides health education and teaching to patient’s, family members and/or significant others, that is age specific, providing for any barriers to learning. Clearly communicates health, treatment and discharge information, ensuring the patient, family member or significant other understands all discharge and follow-up instructions. 10 Effectively performs procedures, such as initiation of IV catheters and fluids, medication administration, dressings, patient teaching, etc., within their scope of practice, and unit, department and Medical Center policies and procedures. 11 Effectively performs other nursing procedures/skills, such as cardiac monitoring, rhythm interpretation, defibrillation, monitoring of conscious sedation, ABGs etc., based completed specialty training and their scope of practice, and unit, department and Medical Center policies and procedures. 12 Appropriately recognizes and quickly responds to emergency medical situations. a) Accurately assesses “A-B-Cs.” b) Initiates appropriate emergency treatment, i.e., CPR, O2, etc. c) Communicates critical information to other team members. 13 Maintains all required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508). 14 Remains current on emergency treatments/practices/procedures through attendance at staff meetings, continuing education events, in-services and the review of professional literature. 15 Integrates/applies new knowledge from education and research into clinical practice. 16 Demonstrates knowledge of and competence in age-related patient care. 17 Consistently demonstrates knowledge of state and federal regulations, such as Title XXII and COBRA/EMTALA. Follow all TJC, CDPH, CMS guidelines. 18 Advocates for public health and safety through patient and family education. 19 Consistently completes required documentation of the Emergency Services visit. 20 Consistently demonstrates caring and compassion through behaviors and interactions in performance of all duties, recognizing the inherent worth and dignity of patients, families, other staff and physicians. 21 Will perform duties of Relief Charge RN if called upon to do so. 22 Adheres to departmental and medical center policies on scheduling, overtime, sick time and time off. 23 Arrives punctually, takes appropriate time for breaks, use sick time appropriately. Qualifications QUALIFICATIONS/JOB REQUIREMENTS: EDUCATION: Graduation from an accredited school of professional nursing. Baccalaureate degree in Nursing preferred. EXPERIENCE: Staff Nurse II – Minimum of six months Registered Nurse experience. Six months current ER & triage experience or completion of Seton ER Nurse Training Program. CERTIFICATIONS/LICENSURE: Active California Registered Nurse licensure Basic Life Support certification (BLS) required ACLS certification required PALS certification required NIHSS certification required Workplace Violence Prevention De-escalation 8-hr (AB508) certification or CPI certification required Preferred: NRP certification. Job Description Summary: An Infusion Nurse II is a licensed registered nurse who coordinates and provides intermediate direct patient care to patients in the home, or in an alternate infusion suite, to ensure patient safety with continuity and compliance under a physician’s plan of care. Effectively and timely communicates with Option Care’s Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Option Care’s policy. Coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care. Demonstrates compliance with agency operations, Option Care’s policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards. Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.
