The professional nurse is accountable to the Director of Patient Care Services for implementing the patient plan of care and communicating each patient needs to the Interdisciplinary team. The professional nurse is also responsible for communication and collaboration with community physicians as well as family members regarding individual patient care. The Hospice Team Nurse provides professional nursing care to patients by utilizing all elements of nursing process. 2. . Assesses and evaluates patient’s status 3. Initiates the plan of care and makes necessary revisions as patient status and needs change. 4. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 5. This position will be TUESDAY-SATURDAY 12PM-8PM. 6. This position will be PRN. 7. This position will be working in different service areas such as the following - Alameda, Contra Costa, Napa, Santa Rosa, Solano and Sonoma. Acts as Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. Communication 1. The Hospice Team Nurse completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. POSITION QUALIFICATIONS 1. Graduate of a National League of Nursing accredited school of nursing. 2. Current licensure in State of CA and CPR certification. 3. Registered nurses / Licensed Vocational nurse shall have a minimum of one (1) year of experience as a professional nurse within the last three (3) years; OR have a baccalaureate degree in nursing from a program accredited by the National League of Nursing and a current RN / LVN license. Minimum of two (2) years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 4. Management experience not required. Responsible for supervising hospice aides. 5. Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist. 6. Prolonged or considerable walking or standing. Able to lift, position and/or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping bending, kneeling and/or crouching. 7. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

The Breast Nurse Navigator coordinates patient care from primary care and imaging services through the multispecialty cancer care team. This role supports timely communication, helps streamline diagnostic and treatment workflows, and serves as a consistent point of contact for patients, referring physicians, radiologists, surgeons, and the multidisciplinary care team. Responsibilities Imaging Center Responsibilities The nurse navigator shares time between the imaging center and the breast specialty clinic. At the imaging center, key responsibilities include: Collaborating with radiologists during biopsy procedures, including obtaining consent, providing post-procedure care, labeling specimens, and completing follow-up phone calls with patients and referring physicians. Obtaining pathology results and communicating findings to referring physicians and patients, as appropriate. Serving as a liaison between primary care physicians and the cancer care team, including helping arrange timely appointments with the multidisciplinary care team. Breast Specialty Clinic Responsibilities At the breast specialty clinic, key responsibilities include: Coordinating consultations with the multidisciplinary care team and serving as a single point of contact for breast cancer patients throughout the care continuum. Helping expedite workups for newly diagnosed breast cancer patients, including coordinating metastatic evaluations when needed. Conducting post-operative phone calls to assess recovery, answer questions, and identify concerns that may require follow-up. Triaging phone calls in the breast surgery clinic and directing patient needs to the appropriate team member. Reviewing results with patients and scheduling follow-up appointments under physician direction. Following patients receiving neoadjuvant therapy, assisting with follow-up appointments, and helping address barriers to treatment. Qualifications Graduated from an accredited school of nursing with a diploma, Associate or Bachelor’s degree in Nursing. Eight years of RN experience, highly preferred. Current California Registered Nurse License CPR certification required. Strong customer service background, preferably in healthcare environment Excellent verbal communication skills Ability to manage and prioritize multiple tasks Proficiency in computer processing functions and other standard office equipment Ability to calmly and professionally resolve customer issues with diplomacy and tact. Process, finalize and facilitate inbound requests that are received from providers. Generate appropriate member and provider communication for all determinations within the required timelines as defined by the most current department policy. Research for appropriate facilities, specialty providers and ancillary providers to utilize for all lines of business. Identification of potential areas of improvement within the provider network. Identify and initiate referrals for appropriate members to the various L.A. Care programs/processes and external community based programs or Linked and Carve Out Services (e.g. DDS/CCS/MH). Potential quality of care/potential fraud issues are identified and documented per L.A. Care policy. High risk/high cost cases and reports are maintained and referred to the Physician Advisor/UM Director. Document in platform/system of record. Utilize designated software system to document reviews and/or notes.
