Job Description BCH Oakland Day Hospital Infusion Center serves the Specialty populations at UCSF Benioff Children’s Hospital. The treatments include but are not limited to apheresis, chemotherapy, biotherapy, immunotherapy, blood transfusions, hydration and electrolyte therapy, injections and blood draws, enzymes and iron infusions, and antibiotics. The center consists of 14 beds, and treatments include research and standard of care infusions. This position requires the incumbent to utilize the nursing process for assessment, planning, implementation and evaluation to provide safe, therapeutic care. Functions as a member of the interdisciplinary and nursing teams; carries out established nursing procedures and performs related duties as required; performs nursing care for patients with complex needs. Manages patient care administered by other members of the nursing team. Recognized as a leader and clinical resource. Qualifications Required Qualifications: Two (2) or more years of recent pediatric nursing experience with at least 1 year in oncology, or 2 years of other directly relevant clinical experience. Job Description Involved in ensuring optimal patient outcomes through effective leadership and / or provision of patient care delivery in assigned nursing unit(s) or area(s). This position requires work as both a UCSF Benioff Children’s Hospital Supervisor and Central Patient Placement (CPP) RN, depending on departmental need. Under the direction of the Director of Hospital Operations, the Benioff Children’s Hospital Supervisor seeks to improve patient care delivery through problem solving and crisis intervention, including disaster management on a broad level. The Hospital Supervisor is recognized as an administrative leader and clinical resource and is accountable for the optimal, safe, effective and timely bed placement of all patients at UCSF Benioff Children’s Hospitals. They utilize nursing process for assessment, planning, implementation and evaluation to assist in the delivery of safe, therapeutic care. While functioning as a CPP RN, they are responsible for effective and timely bed placement for all patients transferring to and located at UCSF Benioff Children’s Hospital. They are responsible for assessing patient level of care placement needs, giving and receiving report. The CPP RN facilitates Pediatric Access Center admissions by screening patients for clinical necessity of the transfer and determining appropriate level of care. Job Description Under general direction of the PBMT Medical Director and in collaboration with the multidisciplinary transplant team, the nurse coordinator provides expert nursing care for transplant, cell and gene therapy patients, including candidates (pre-transplant), recipients (post-transplant) and/or donors with a variety of complex treatment options. The nursing coordinator serves as a contact point for transplant patients, family members, referring physicians, staff as well as contact point for the coordination and facilitation of the care of the patient in specialty areas. The nurse coordinator provides leadership for collaborative and multidisciplinary patient care with emphasis on education and continuity of patient care for the entire period of care including inpatient, outpatient, or both. The transplant nurse coordinator is responsible for monitoring and coordinating the care of transplant patients to ensure they reach their maximum health potential before and/or after transplant. Specific daily responsibilities include triaging patient calls and test results, coordinating organ evaluation studies and specialty referrals, confirming orders, providing ongoing transplant patient education, spearheading the coordination of care for the transplant patient across disciplines, resourcing and assisting nursing with cellular therapy infusions, coordinating discharge care, and clinic work as needed. This position will support the transplant team by communicating with patients on behalf of providers. Respond to changes in patient condition as exhibited in signs, symptoms and lab values in collaboration with the provider. The nurse will make changes to medication and treatment plans as delegated by the provider. The nurse coordinator will educate patient on any changes and document in the EHR. Lastly, they will prep and review charts for providers for patient pre and post-transplant ambulatory visits.
