Position Summary: We have an exciting opportunity to join our team as a Registered Nurse, Per Diem - MICU (2 yrs min exp required). Considers needs and behaviors of specific patient age groups and cultural groups in all patient care activities. Documents interventions per hospital standard. Educates the patient/family regarding health status and established plan of care. Determines learning needs of the patient/family. Implements an educational plan. Collaborates with the interdisciplinary team to modify expected outcomes. Documents learning outcomes of patient/family and revises education plan. (Include utilization of EHR education tools) Evaluates the patient's progress towards expected outcomes. Involves the patient/family and interdisciplinary team in the evaluation process, recognizes changes in patient condition, reports documents and evaluates the effectiveness of the intervention(s). Participates in and contributes to in unit activities to provide patient care in a healing, humane and caring environment. Demonstrates understanding of and contributes to unit throughput efficiencies and metrics. (Include unit throughput measures e.g. DBN, door to triage, assign to occupy, first case starts, room turnover) Complies with unit based and organizations professional standards, New York State Nurse Practice Act and regulations of relevant outside agencies, e.g., NY State Department of Health, The Joint Commission, regulatory requirements etc. Exhibits appropriate judgement in delegated activities. Minimum Qualifications: To qualify you must have a Current Registered Professional Nurse in New York, Bachelors degree in Nursing from an accredited institution required, Basic Life Support Certification through the American Heart Association, Fluent written and spoken English language skills.. Job Summary The Assistant Nurse Manager (ANM) – Critical Care Services is responsible for assisting in the coordination, oversight, and evaluation of nursing and clinical operations within the critical care environment. This role supports the Nurse Manager in ensuring the delivery of safe, evidence-based, high-quality, and patient-centered care across the assigned Nursing departments. In collaboration with physicians, nursing leadership, and the interdisciplinary team, the ANM assists in identifying at-risk patients, coordinating care plans, and ensuring optimal patient outcomes. Operational & Administrative Support Assists Nurse Manager with daily operational management including staffing coordination, scheduling, and assignment adjustments based on acuity and staffing needs. Supports payroll review and validation to ensure accuracy and compliance with organizational policies. Participates in chart audits, documentation quality reviews, and compliance monitoring. Assists in policy and procedure development, revision, and implementation in alignment with regulatory and evidence-based standards. Answers patient questions and gives instructions on tests and procedures that are to be performed; attempts to reduce patient's anxiety and fear. Scheduling patient appointments on computerized system. Coordinates assignments to maintain patient flow throughout clinic. Description A nurse practitioner (NP) as described in Title VIII, Article 139 section 6910 of the New York Education Law is a RN who has earned a separate certification as an NP through advanced clinical nursing education in a distinct specialty area of practice. Description Participates in the application of the nursing process to an assigned group of patients. Participates in the application of the nursing process to an assigned group of patients. 2. Administer IV fluids, provided completion of annual competency IV therapy training. 6. Monitors patients receiving blood and blood products and TPN under the direction of the RN, who administers the transfusion. 7. Makes recommendations in the application of the nursing process. 8. Participates in planning, and administers direct patient care, including activities of daily living (ADL). 9. Performs point of care testing including blood glucose. 10. Performs nursing procedures as defined in the nursing procedure manual. 11. Administers medication as defined in nursing policy. ALL routes except cannot administer medications IV push/bolus (except for a Normal Saline or heparin flush) or via a central line. LPNs do not administer chemotherapy or immunotherapy. 12. LPNs cannot administer first dose of an IV drug. The RN will administer the first dose of an IV drug and assess for side effects, after which the LPN can administer subsequent doses. 13. LPNs working exclusively in chronic outpatient hemodialysis settings may, under the direct supervision of a Registered Professional Nurse (RN) and after completing an approved hemodialysis training and competency program, perform dialysis-related tasks as permitted by the New York State Education Department (NYSED) Office of the Professions and 10 NYCRR §700.4(b). See Section D for more details regarding role of LPNs in the Outpatient Hemodialysis setting. This exception does not permit independent practice or unsupervised access to central venous lines. 14. Assists in patient assessment after the RN has completed the initial patient assessment, including pain management. 15. Carries out medical and nursing orders. The training should include a written examination and competency demonstration. A Hemodialysis LPN may: • Administer bolus of medications by IV push in a chronic outpatient hemodialysis treatment center, excluding first dose administration which is administered by an RN. The RN will access and care for patient’s initial catheter use. Subsequently, and following the MSHS CVAD care policy, the LPN can: • Identify and set up supplies necessary for accessing a CVAD. • Initiate treatment with a CVAD. • Draw blood specimens from a CVAD. • Perform CVAD dressing change and exit site care. • Monitor and adjust flow rates of central venous catheter access device/line. • Intervene if there is a catheter/blood line disconnection or other complications associated with a CVAD. • Provide CVAD care at the end of dialysis treatment. Description The Medical Assistant I provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. This individual is also responsible for assisting with clinical, administrative and organizational tasks associated with patient care coordination. Responsibilities 1. Assists the provider during patient visit via obtaining vital signs (height, weight, blood pressure, pain scale etc) and documenting vital sign results and transcribing medication information into the electronic medical record (EMR). 2. Obtains prior authorizations for prescriptions and durable medical equipment 3. Drafts prescription requests in EMR and sends to provider for review, completion and processing 4. Performs Phlebotomy services inclusive of collecting and processing blood, urine and stool specimens and use of a centrifuge for spinning bloods 5. Performs POCT (w/the exception of glucose) and documents results in EMR for provider to review 6. Performs daily Quality Control for all POCT equipment 7. Orders clinical supplies when necessary 8. Maintains and stocks exam and consult rooms with clinical supplies and clerical forms used by Providers 9. Assists providers with procedures by preparing supplies and equipment and acting as a chaperone as needed. 10. Perform EKGs when ordered by provider 11. Performs pre-visit planning for Diabetic and CHF patients as per practice guidelines 12. Monitor and maintains refrigerator temperature logs and biomed expirations for medical equipment 13. Participates in daily huddles with MDs and other clinical team members to review daily care management patient lists and discuss patient's treatment plan & management 14. Performs age-specific screenings for Activities of Daily Living, Falls, Alcohol, Tobacco, and/or Depression screenings annually for every patient 15. Enters data into the electronic medical record related to medication adherence, diabetes control, foot/eye exam compliance for provider review and reconciliation 16. Assists providers with distributing educational materials to patients 17. Assists Population Health Management team by verifying patient has completed their colorectal screening, breast screening, cervical cancer screening and other pertinent preventive screens. 18. Retrieves information regarding hospital admission and Emergency Room visits from outside facilities for evaluation by provider for transition of care coordination 19. Assists providers with follow-up related to patients on Coumadin therapy 20. Assists at the front desk as needed: answer telephones, register patients, discharge patients, schedule appointments, copy charts, schedule ancillary tests, and follow up on overdue diagnostic test results. 21.
