188109 Job Description Has knowledge of each facility's discharge planning process to accept and refer patients appropriate for skilled home care services. Under supervision, formulates and implements a Home Care Plan in conjunction with the physician, patient, patient’s family and discharge planner. Position schedule is M-F, 9:30-5:30p with weekend rotation, 100% on site in a Garden City office. Job Responsibility Responds to referrals in a timely manner and evaluates patients for appropriateness of skilled home care services. Plans for the skilled home care needs of patient in a timely manner to ensure patient’s expeditious movement to the appropriate level of care. Implements a comprehensive home care plan based on the in-depth assessment of the patient’s needs, including coordination of activities with family, informal supports and other community resources. Maintains working knowledge of third-party reimbursement guidelines and regulation standards. Functions as a home care service resource person. 182289 Job Description Provides physician-supervised physical and emotional care for the mother during labor, delivery, and postpartum phases. Evaluates progress and manages the labor and delivery. Evaluates and provides immediate care for the normal newborn. Job Responsibility Performs complete history and physical examination on new patients admitted to the labor room, and writes admission notes utilizing the attending physician when appropriate. Discusses new admissions with the attending physician and nursing staff including noted complications. Decides who will follow the patient and develops a plan for care. Performs admissions diagnostics and care including, but not limited to pelvic examination, cervical cultures, drawing bloods, and starting intravenous lines. Follows progress of labor of all patients in labor and delivery suite in collaboration with the attending physician and nursing staff. Monitors fetal heart rate patterns and notifies the appropriate physician of significant deviations. Examines patients periodically to determine progress in labor with ongoing assessment of maternal and fetal reaction to the progress of labor. Reports any abnormalities in patient’s condition to physician including, but not limited to meconium, bleeding, hypertonic labor, fetal distress, failure of progress in labor, malpresentation.
