This position will focus exclusively on inpatient/floor-based vascular surgery care, working collaboratively with vascular surgeons and the multidisciplinary healthcare team to provide high-quality care to hospitalized vascular patients. The APP will be responsible for the evaluation and management of vascular surgery patients throughout their inpatient stay, including admissions, daily floor management, pre-operative and post-operative care, discharge planning, and coordination of care. Position Focus This is an inpatient/floor-based APP position. The role does not include routine OR or surgical-assistance responsibilities. The APP will primarily focus on vascular surgery admissions, inpatient consults, daily floor management, pre-operative and post-operative care, clinical assessment, discharge planning, and coordination of care in collaboration with the vascular surgery team. Responsibilities 1. Inpatient Patient Evaluation & Management Perform comprehensive histories and physical examinations on vascular surgery patients. Evaluate new inpatient vascular surgery consultations and assist with developing appropriate treatment plans. Perform daily patient assessments and manage the ongoing care of vascular surgery patients on the floor. Monitor patients for changes in clinical status and promptly identify potential complications. Monitor surgical wounds, drains, vascular status, laboratory results, and overall recovery. Recognize and appropriately manage post-operative complications in collaboration with the vascular surgeon. Manage pain, medications, fluids, and other aspects of routine inpatient care within the scope of practice. Provide patient and family education regarding treatment plans, recovery, and discharge needs. 3. Inpatient/Floor Responsibilities Perform daily rounds with the vascular surgery team. Write daily progress notes, admission notes, consult notes, discharge summaries, and other required clinical documentation. Place appropriate orders and manage routine inpatient needs. Coordinate consultations with other medical and surgical specialties as needed. Communicate with nursing, case management, physical therapy, social work, and other members of the multidisciplinary team. Collaboration & Communication Work closely with vascular surgeons to provide coordinated inpatient care. Qualifications Graduate of an accredited Physician Assistant or Nurse Practitioner program. Current New York State licensure as a PA or NP. Overview Provides direct psychiatric evaluation, medical health assessment and intervention to clients admitted to Behavioral Health programs. Provides medication management and assessment on an ongoing basis. Provides consultation and education to other agencies and disciplines, including other VNS Health services. Assists in the overall management of the Behavioral Health program. Acts as a key resource in providing clinical and operational guidance and support to assigned teams and other staff to achieve and enhance team outcomes. Evaluates client responses to therapy and interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes. • Consults with other mental health providers and community agencies, including other VNS Health department/programs, as well as members of the Behavioral Health program, regarding individual clients. • Participates in interdisciplinary team meetings and case conferences for the Behavioral Health program. • Maintains client records in compliance with VNS Health, state, city and federal requirements. • Evaluates clients for psychiatric hospitalization, as necessary. • Provides clinical supervision and direction to Behavioral Health program staff, including on-site observation of direct patient care and participation as needed in direct care. Participates with Behavioral Health program staff and other appropriate VNS Health staff in the development and implementation of in-service trainings. • Collaborates with program management to ensure the provision of the highest quality behavioral health services to clients.
