Job Overview The position is a member of the hospice staff who determines appropriateness of referrals, coordinates and conducts pre-admission activities, completes admission when determined hospice appropriate , monitors inpatient care at contracted hospitals, performs in-service training and community education activities. Reviews and explains to patient and/or family about insurance coverage and financial obligations, using knowledge of hospice fees, costs and reimbursement sources. Escalates to referral management director and/or referral manager, in circumstances of potential admission where reimbursement sources appear not to cover hospice fees, to ascertain whether reduce fee or free care may be offered. Introduces and explains benefits of hospice care and familiarizes decision makers with characteristics and needs that indicate an appropriate hospice referral. Presents the advantage of the hospice Medicare/Medicaid benefits to referral sources. Thoroughly understands the Medicare/Medicaid hospice benefit, the Medicare home health benefit, the Medicare/Medicaid long term benefit and the Medicare HMO benefit. Documents a narrative note in the EMR summarizing pertinent physical and psychosocial information from the hospital record and/or from the physician and patient/family assessment. Communicates information from Comprehensive Assessment to TMCAH CMO, hospice physician or TMCAH covering NP. Documents results and performs warm hand off to team. Job Description: I. Position Summary: The primary role of the Patient Care Associate (PCA), is to protect, sustain, and nurture the patients that live at the Center. Whittier Rehabilitation Hospital, Bradford/Haverhill Wound Care Specialist - CWCN/CWOCN $50 - $60 per hour plus $10,000 Sign-On Bonus Full-Time Days, 40 hours/week Monday-Friday, flexibility available We are looking for a Certified Wound Care RN to join our nursing team within our beautiful Rehabilitation Hospital conveniently located off of 495 on Ward Hill in Bradford Mass. Essential Duties: -Acts as a role model for professional practice in wound care (in outpatient wound clinic and inpatient as necessary) -Provides and/or coordinates patient care, including appropriate referrals for required services and necessary equipment to meet the wound care needs of the patient. -Participates in interdisciplinary meetings as appropriate. -Evaluates equipment needs and makes recommendations to the Director of Nursing and Clinical Services appropriately. -Is responsible for equipment management by providing education and monitoring safety. -Collaborates with affiliating agencies to meet the care needs of the wound care patients -Initiates a nursing assessment of patient's condition upon admission to the wound clinic. -Assesses and interprets reports communicating findings with the assigned physician. -Recognize and assesses initial alterations in the patient's body system and verbalizes this to other members of the health care team. -Organizes assessment data for accuracy, completeness and confidentiality -Records assessment data in an orderly fashion and communicates, revises, and verbalizes pertinent information to other health care team members. -Assesses, stages, measures wounds accurately and in accordance to policy. - Photographs wounds according to policy and procedure -Identifies patient care situations that require intervention, implements nursing actions and evaluates responses. -Provides consultation to nursing staff and other health care professionals. -Provides leadership in the development, implementation and evaluation of standards of practice within specialty area. -Collaborates with Director of Nursing, Clinical Services and the Coordinator of PI to identify learning needs of the staff.
