Torquing: Not required. Controls & Equipment: Blood pressure gauge, thermometer, etc. Other Physical Considerations: Twisting: Occasionally may occur while transferring patient. Bending: Occasionally occurs while taking vital signs, assisting patient in bathing, providing peri-care, etc. Squatting: Occasionally may occur while bathing patient; also required when putting on shoes and socks of patients, and also when making eye-to-eye contact while talking if patient is in a wheelchair. Kneeling: Occasionally may occur while bathing patient. Crouching: Required when putting on shoes and socks of patients, and also when making eye-to-eye contact while talking if patient is in a wheelchair. Climbing: Not required. Balancing: Not required. May be subject to hostile and emotionally upset patients, family members, associates and visitors. OTHER CONSIDERATIONS AND REQUIREMENTS In this position, the associate is required to be on his or her feet continually throughout the day. In a full-time nonrestrictive basis, the associate must be able to lift at least 40 pounds of weight, which is required when transferring a patient from a bed to a wheelchair. A transfer belt is necessary and required when transferring any patient. This is for the safety of not only the patient, but also the aide. Other nurses are available to assist with heavier patients as needed. Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Bergen/Morris County. Responsibilities Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team. Provide guidance, support, and mentorship to RN Case Managers and other clinical staff. Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage. Monitor patient care to promote continuity, quality, and compliance with the hospice plan of care. Assist nurses with clinical questions, challenging patient situations, symptom management, and changes in patient condition. Participate in patient care and make nursing visits as needed to support the clinical team. Assist with reviewing clinical documentation for completeness, accuracy, and compliance. Help monitor timely completion of assessments, recertifications, care plans, orders, and other required clinical documentation. Assist with identifying changes in patient condition and ensuring appropriate communication with physicians, hospice providers, patients, and families. Support coordination between nursing, aides, social work, chaplain, volunteers, providers, and other members of the interdisciplinary team. Serve as a clinical resource and role model for nursing staff. Participate in interdisciplinary group meetings and other clinical meetings as assigned. Respond appropriately to urgent clinical situations and assist with escalation of patient-care concerns. Qualifications Current, unrestricted Registered Nurse (RN) license in the State of New Jersey. Associate Degree in Nursing required; BSN preferred . Minimum of 2–3 years of professional nursing experience . Previous hospice, home health, palliative care, or end-of-life care experience strongly preferred. Previous leadership, charge nurse, preceptor, or supervisory experience preferred. Current CPR/BLS certification. Valid driver's license, reliable transportation, and appropriate automobile insurance. Strong clinical assessment and critical-thinking skills. Excellent written and verbal communication skills. Strong organizational and time-management abilities. Ability to work independently while functioning effectively as part of an interdisciplinary team. Proficiency with electronic medical records and standard computer applications. Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. Job Description The Registered Nurse in the Sexual Health Center provides direct patient care, coordinates treatment plans, collaborates with interdisciplinary teams to ensure quality outcomes, maintains clinical documentation, adheres to safety protocols, and supports patient education and advocacy. The RN will work partly in the ambulatory fixed site at 200 South Street in Morristown, partly in the Morristown Medical Center Emergency Department, and partly in the community in an Atlantic Mobile Health vehicle and at community-based organizations through Northern New Jersey including, but not limited to, Morris, Sussex, and Passaic Counties. The Sexual Health Department in a small, interdisciplinary team that primarily promotes public health initiatives funded through grants and awards. Public health initiatives include prevention, testing, and treatment for HIV, HCV, and sexually transmitted infections; harm reduction health programming such as preventative care, acute and chronic disease management, and wound care; education and counseling to promote safer sex and use strategies. Respond appropriately to emergencies and changes in resident condition. Participate in admissions, transfers, and discharges. Collaborate with rehabilitation, dietary, social services, and other departments to ensure comprehensive resident care. Educate residents, families, and staff regarding care plans and health conditions. Maintain compliance with New York State Department of Health regulations and facility policies. Participate in quality improvement initiatives and staff education programs. Qualifications Required: Current New York State Registered Nurse (RN) License. Graduate of an accredited nursing program. Current CPR/BLS Certification. Excellent communication and organizational skills. Ability to work collaboratively in a fast-paced healthcare environment. Preferred: Long-term care or skilled nursing experience. Experience with electronic medical records (EMR). Knowledge of CMS regulations and quality measures.
