About the Role We are currently looking for an experienced Medical Assistant to join our team. The Medical Assistant is a key component of our care team and works closely with a primary care physician to deliver excellent patient care and provide an exceptional guest experience to our patients. Because of our unique model, our Medical Assistants oversee the full spectrum of the patient experience- acting both as a Medical Receptionist as well as assuming clinical responsibilities of a Medical Assistant. Essential Duties and Responsibilities The following duties and responsibilities generally reflect the expectations of this position but are not intended to be all-inclusive. Escorts patients to room and ensures that exam rooms are thoroughly cleaned and stocked Reviews previous medical information and gathers any relevant updated health information from the patient to inform the provider Take vitals, perform blood draws, give injections, bandage wounds, assist with procedures. Work with the contracted labs for additional tests Prepares and administers approved medications and immunizations via oral, topical, inhaled, intramuscular, subcutaneous or intradermal at the direction of and upon written order from the Provider Partners with other members of the care team to develop individual patient plan of care including identifying and working to address gaps in care Front desk duties, including: patient scheduling, medical record requests and abstraction, documentation, coordinate specialist visits, referrals, and other care outside of the clinic, check-in, check-out, etc. Records accurate and pertinent data in the medical record according to documentation guidelines Process test results and provide the patient with timely results via e-message, mail or phone as directed in written instructions by the Provider Participates in outreach to patients to drive engagement among eligible patient groups Maintains clinical and office supplies and equipment for treatments About You Minimum of 1 year of experience working as a Medical Assistant, preferably in Primary Care. May vary based on clinic needs Graduation from a formal Medical Assistant program or other related program National or state-specific MA certification or registration is strongly preferred. MA certification or registration is required for employment in States where certification/registration is required. CPR/BLS certification required at time of start date Phlebotomy experience is preferred 
Job Summary: The Registered Nurse will administer medication to perform moderate sedation; participate in diagnostic and treatment procedures in SUNY Upstate Electrophysiology Lab. Responsibilities in this role will include but not limited to: Documenting activities of patient care and testing. Assist physicians in testing and treatment of cardiac arrhythmias in the Electrophysiology lab. Communicate results of testing to cardiologist/electrophysiologist, nursing and case managers. Continuing education research and patient teaching. RN will also participate in patient care and unit activities of HVC departments of Cath Lab, Cardiac Rehab and ECHO Lab. Minimum Qualifications: Associate's degree in Nursing or equivalent, NYS RN licensure (or eligible) with a minimum of 2 years of Cardiac Care nursing experience (preferably in an ICU/critical care setting). Certification in CPR/BLS required at time of appointment. Must be able to handle physical aspects of patient care such as moving patients and equipment as well as participate in rotating on-call schedule. Job Summary The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum. Job Description GENERAL SUMMARY Our Patient Care Advocate are the energetic, curious explorer, part of a team that provides compassionate care to the frailest elderly who suffer from chronic and acute illnesses or injuries living in their homes. Our PCA is responsible for developing a rapport, and delivering exceptional in-home services, helping coordinate care with the rest of the medical team, scheduling patient appointments with our team, specialist appointments and coordinating diagnostic tests, HEDIS gap closure, being a community resource, working to engage patients that currently are not engaged, and contributing complete and accurate data in patient's records, and other tracking tasks.
