We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care. Job Description Harborview Medical Center Inpatient Acute Care Float APP (ARNP; Nurse Practitioner; or PA-C, Physician Assistant) This opening is for 100% FTE, Days; (and with weekend and holiday coverage, and with the potential for some night shifts). The Inpatient Acute Care Float APP team is composed of licensed physician assistants and nurse practitioners who are skilled and trained to work across multiple services at Harborview (currently including General Surgery, Medicine Polytrauma Service, Neurosurgery, Orthopedic Surgery, & Burns). Float APPs possess the ability to adapt quickly to different clinical environments and provide comprehensive care. They play a critical role, filling in when there are staffing shortages, leaves and fluctuating demands. Work is performed 7 days a week with general independence and decision-making responsibility under the general guidance of the attending physicians or fellows, lead APP and APP assistant director. Guidelines include general policy procedures and practice guidelines established by the department and Harborview Medical Center, as well as appropriate professional and technical manuals. Roles & Responsibilities Practice high quality evidence-based medicine within the scope of a licensed APP in the inpatient acute care setting including: Rotating among inpatient services including but not limited to General Surgery, Medicine Polytrauma Service, Neurosurgery, Orthopedic Surgery, & Burns Conduct comprehensive patient assessments and physical examinations Diagnose and manage acute & chronic medical conditions Develop and implement individualized treatment plans Monitor and evaluate patient progress and adjust treatment plans as necessary Perform procedures within one’s scope and training as appropriate Order and interpret diagnostic tests Prescribe and manage medications Coordinate patient care with multidisciplinary teams, fostering a cooperative atmosphere that promotes effective communication and teamwork Provide patient and family education Engage in patient discharge planning and follow-up care coordination Document patient care and maintain accurate medical records Participate in quality improvement initiatives and safety protocols Complete learning modules and participate in continuing education Provide mentorship and guidance as appropriate to other staff and students Requirements Nurse Practitioner: Masters or Doctorate Degree in nursing from an accredited school of nursing AND Washington State Advanced Registered Nurse Practitioner License with prescriptive privileges AND National certification as an ACNP (Acute Care Nurse Practitioner); (FNPs/Family Nurse Practitioners with at least 3 years of APP/provider level experience in acute care will also be considered).
