Registered Nurse Supervisor 1 (T&RC): supervisory level; provides administrative and clinical supervision to lower-level registered nurses and direct care staff; oversees and coordinates nursing services for an assigned unit; and may be involved in teaching and orientation functions. Related Classes Teaching & Research Center Licensed Practical Nurses are licensed practical nurses that provide direct nursing care to individuals receiving treatment in State University of New York teaching and research center hospitals under the direction of a registered nurse, physician, or other licensed health care provider. Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Participates in the development, implementation, and revision of individual care plans as a member of a multidisciplinary treatment team. Modifies nursing care plans based on individual condition and responses to care and treatment. Triages individuals by inquiring about illnesses, injuries, and symptoms, and records findings and observations. Assesses findings, and either refers individuals directly to a physician in emergency cases or initiates appropriate remedial action; as directed, assists physicians with treatment functions. Provides education and counseling to individuals and families regarding health, medications, procedures, and treatment goals. Registered Nurse 1 (T&RC) Independently performs the duties described above and provides clinical supervision and instruction to Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff in completing care tasks. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. May perform the full range of supervisory duties over Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors 1 (T&RC); and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Registered Nurse 2 (T&RC) Positions perform duties in each of the three functional areas described below. Direct Individual Care: Provides nursing care to individuals in an acute care environment requiring complex nursing skills; uses advanced knowledge and skill in nursing assessment, planning, implementation, and evaluation activities. Observes and responds to individual emergency situations, and sets priorities for care based on individual acuity, preferences, and the possibility of effective intervention. Implements nursing interventions in response to changes in individual status and unusual circumstances. Interprets assessment data to determine if individuals are at physiological or psychosocial risk. Identifies implications of individuals’ statuses and prioritizes data collection based on individuals’ immediate needs. Identifies safety and security risks to promote a culture of safety and to prevent injury and harm to individuals, visitors, and staff. Preceptor: Guides lower-level, and new to specialty, registered nurses in choosing nursing interventions, developing skills, and applying knowledge. Serves as a resource nurse for staff members, orienting new staff and providing feedback on nursing skills for specialty care areas. Develops and maintains care standards knowledge bases for specialty care areas and develops care guidelines for unit-specific individual populations. Maintains a minimum standard of continuing education for specialty areas. Evaluates and assesses individual staff and nursing student development needs. Counsels and implements a variety of teaching strategies to meet those needs in consultation with nursing management staff. Participates in in-service and other educational programs related to clinical specialties. May identify, develop, and implement nursing research studies on clinical units, in consultation with medical staff consultants. Clinical Management: Consults and coordinates with other health care team members to resolve complex clinical issues and develop short and long-term clinical goals for assigned units. Integrates and evaluates clinical care knowledge specific to the needs of unit individuals; develops unit-specific policies and procedures in consultation with nursing management staff. Identifies and implements corrective actions to improve the quality of individual care; may oversee equipment testing and supplies. Leads and attends various hospital committees. Assists Registered Nurse Supervisor 1 (T&RC) staff with implementing and achieving unit operating changes; may take charge of a unit in the absence of a Registered Nurse Supervisor 1 (T&RC). Resolves individual relation issues. Identifies and reports issues related to individual care, operation, and quality outcomes. Identifies potential conflict situations and facilitates solutions. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Illustrative Duties Nursing Assistant 1 Under Direct Clinical Supervision, Performs The Following Duties Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Applies abdominal “T” binders, hot water bottles, aqua “K” pads, ice bags, and coolers, and hot and cold compresses to patients as directed. Assists patients in collecting urine and stool specimens. Observes and records patients’ quantitative food and fluid intake and waste output. Takes temperature, pulse rate, blood pressure and respiration rate. Gives therapeutic baths such as sitz and fever sponge baths. Measures and records height and weight. Inspects functioning oxygen therapy equipment including tents, masks, and cannulae to ensure appropriate functioning. Recognizes and reports abnormal signs and symptoms of common diseases and conditions to clinical staff. Applies splints, braces, slings and other self-help devices. Operates a variety of specialized patient monitoring and treatment devices. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. Overview Provides mental health assessment of and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Provides support and education to VNS Health staff and caregivers to enable them to better care for patients with psychiatric, cognitive and/or behavioral problems. Works under moderate supervision. • Conducts psychosocial assessments and living conditions of patients and develops treatment recommendations. • Travels to patients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. • Transports and utilizes required medical equipment and supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient homes/care facilities, VNS Health offices and other locations. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • Provides nursing care in accordance with Agency policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient care. • Provides any of the following services to patients: advocacy, psycho-education, cognitive/behavioral interventions, monitoring of mental status, emotional support, brief psychotherapy, family based interventions. • Provides crisis intervention as necessary. • Consults with Program Coordinator as necessary to develop psychiatric evaluation and treatment plan. • Consults with referrer on assessment, recommendations, and update on progress of treatment. • Provides education, consultation and support to VNS Health staff. • Assist the health care provider in implementing the plan of care. • Follows a treatment plan, revising and updating as necessary. • Makes appropriate referrals for mental health services. • Participates in interdisciplinary team meetings, case conferences and clinical supervision. • Attends in-service education programs. • Assumes responsibility for continued professional growth. • Maintains productivity sufficient to meet VNS Health Home Care standards. • Maintains records in accordance with AMH and VNS Health record keeping standard and performs established workflow procedures. • Participates in special projects and performs other duties as assigned. Manages implementation of clinical staff development programs such as orientation, in-services, training and other learning activities. Ensures that development and delivery of programs meet the strategic objectives of specific programs. Ensures all clinical education programs are in accordance with standards of nursing and social work practice, state and federal regulations and VNS Health policies and procedures. Reviews program specific educational program outcomes and program evaluations to determine areas for improvement and makes recommendations for change and/or new programs for clinical education as appropriate. Presents improvement strategies to department leadership. Trains and/or oversees training of new and current staff in VNS Health/program philosophy, policy, standards, procedures, documentation, models for service delivery and efficient, productive, cost-effective work processes. Monitors and evaluates performance of newly hired staff and staff referred on for remedial or additional training. Counsels, disciplines, terminates or extends probation for staff based on performance during introductory, probationary or training period. Designs new and modifies existing educational curriculum and training materials to ensure staff has current knowledge and competencies to meet program contractual and regulatory compliance. Conducts educational needs assessment in collaboration with operations staff to identify learning needs among staff. Analyzes and utilizes assessment data to plan for program educational sessions. Develops education programs such as workshops and/or group learning activities; follows up to ensure programs are effectively implemented. Identifies performance issues and develops performance improvement plans for tenured employees in collaboration with program management. Evaluates competency and performance in conjunction with program management. Reviews and evaluates clinical orientation and ongoing educational program outcomes to determine areas for improvement and recommends changes to current orientation program as appropriate. Participates in the development of revised programs and/or materials. Communicates ongoing e-learning opportunities. Evaluates e-learning usage and effectiveness; identifies opportunities for distance learning. Analyzes course evaluations in order to evaluate effectiveness of training sessions and to implement improvements. Participates in the development of annual clinical education strategic goals and objectives. Utilizes resources and practices sound fiscal responsibility; provides input for the development of the department budget. Monitors expenditures and ensures compliance with budgetary constraints. Collaborates with Quality department in the development of and implementation of education related training programs. Ensures program content is presented in a manner that promotes learning and enables trainees to meet competency requirements. Demonstrates services excellence and creates and implements education and training programs which are centered on the goals and strategies of VNS Health. Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Coordinates medical and nursing care needs with other health care providers and departments. Ensures proper functioning of equipment and unit compliance with safety and environmental standards. Monitors nursing care quality standards, and implements corrective actions as needed.
