Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Nursing Assistant 2: full performance level; using detailed guidelines, standards, and procedures, and under the supervision of clinicians, observes patients and performs prescribed simple treatment procedures; attends to personal hygiene needs of patients; and performs limited cleaning and housekeeping duties directly related to patient care. Treatment tasks performed require only knowledge of a limited number of specific treatment procedures performed at the facility. Nursing Assistant (Certified): full performance level; performs duties similar to a Nursing Assistant 2, but functions are performed in a long-term care facility and require certification as a New York State Certified Nursing Assistant. Related Classes Rehabilitation Hospital Nursing Assistants perform similar functions to those of Nursing Assistants but are focused on rehabilitation activities and are only classified at DOH’s Helen Hayes Hospital. Licensed Practical Nurses are classified in a variety of State health care settings and observe patients for symptoms and reactions to treatments; treat patients for physical maladies as directed and under close supervision; and attend to patients’ personal hygiene needs. Illustrative Duties Nursing Assistant 1 Under Direct Clinical Supervision, Performs The Following Duties Changes simple dressings as directed, gives enemas, douches, and irrigations as directed. Applies abdominal “T” binders, hot water bottles, aqua “K” pads, ice bags, and coolers, and hot and cold compresses to patients as directed. Assists patients in collecting urine and stool specimens. Observes and records patients’ quantitative food and fluid intake and waste output. Takes temperature, pulse rate, blood pressure and respiration rate. Gives therapeutic baths such as sitz and fever sponge baths. Measures and records height and weight. Inspects functioning oxygen therapy equipment including tents, masks, and cannulae to ensure appropriate functioning. Recognizes and reports abnormal signs and symptoms of common diseases and conditions to clinical staff. Applies splints, braces, slings and other self-help devices. Operates a variety of specialized patient monitoring and treatment devices. Attends to patients’ hygiene and daily living needs in order to maintain an atmosphere conducive to patient comfort and treatment. Examples include bathing and dressing patients, brushing teeth, and trimming nails. Collects and carries patients’ laundry and dry cleaning and delivers objects to appropriate locations in the health care facility. Receives patients’ belongings and transports objects to safe storage, keeping appropriate records. Feeds patients requiring assistance. As directed by clinical staff, moves, positions, lifts and transports patients using a variety of lifts and other aids in order to make patients comfortable, prepare patients for treatment, deliver patients to treatment areas, and to prevent contractures. Brings bedpans and urinals to patients requiring assistance and attends to incontinent patients. Talks and listens to patients and responds by encouraging patients to cooperate and participate in treatment activities. May transport patients and supplies on occasion. Nursing Assistant 2 Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 1, but with greater proficiency. Positions may assist with training new staff in assigned treatment areas. Nursing Assistant (Certified) Under direction of higher-level treatment staff, performs duties similar to those of a Nursing Assistant 2, but also participates in required quarterly in-service education to maintain certification and meet all necessary requirements to be re-certified every two years. Communication Nursing Assistants have frequent face-to-face communication with patients and visitors for the purposes of conveying treatment procedure and visitor information. Incumbents attempt to encourage and support patients who may be despondent or apprehensive, and regularly communicate with clinical staff to convey information on patients’ physical conditions and responses to treatment. This information includes items such as observable physical behavioral changes, fatigue levels, and general behaviors and responses to routine treatments, such as foot soaks. Description As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Overview Provides mental health assessment of and treatment to patients with psychiatric symptoms, cognitive deficits, and/or behavioral problems. Description Medical Assistant II-Multispecialty-Brooklyn, NY-Full Time-Day-Offsite Provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. Levels in the series are primarily distinguished by scope and independence of operation, which is based on incumbents’ credentials (i.e., possession of a limited permit as a graduate nurse or eligible to enter examination for licensure as a registered professional nurse; or possession of a license and current registration as a registered professional nurse in New York State). Graduate Nurse (T&RC): entry level; possesses a limited permit as a graduate nurse or is eligible to enter examination for licensure as a registered professional nurse in New York State; under the close supervision of a higher-level registered nurse, provides direct nursing care to individuals in accordance with Article 139 of the Education Law. Related Classes Teaching & Research Center Licensed Practical Nurses are licensed practical nurses that provide direct nursing care to individuals receiving treatment in State University of New York teaching and research center hospitals under the direction of a registered nurse, physician, or other licensed health care provider. Illustrative Duties Graduate Nurse (T&RC) Prepares or assists with preparation of nursing care plans consistent with individuals’ comprehensive treatment plans and medical needs. Carries out treatment procedures, administers medications, and monitors individuals’ conditions. Records notes and maintains individuals’ medical records and nursing care plans, including nursing care, medications, and treatments, reactions to care and treatment, intake and output of liquids and solids, and vital signs. Participates in the development, implementation, and revision of individual care plans as a member of a multidisciplinary treatment team. Modifies nursing care plans based on individual condition and responses to care and treatment. Triages individuals by inquiring about illnesses, injuries, and symptoms, and records findings and observations. Assesses findings, and either refers individuals directly to a physician in emergency cases or initiates appropriate remedial action; as directed, assists physicians with treatment functions. Delegates responsibility for appropriate nursing activities, skills, and/or procedures to nursing and direct care staff; and makes clinical rounds to assess individuals’ nursing care needs and observe and evaluate the provision of nursing services. May perform the full range of supervisory duties over Graduate Nurses (T&RC), Teaching & Research Center Licensed Practical Nurses, and direct care staff; provide information to Registered Nurse Supervisors 1 (T&RC); and prepare or contribute to the preparation of reports and performance evaluations for lower-level staff involved in nursing care activities. Registered Nurse 2 (T&RC) Positions perform duties in each of the three functional areas described below. Direct Individual Care: Provides nursing care to individuals in an acute care environment requiring complex nursing skills; uses advanced knowledge and skill in nursing assessment, planning, implementation, and evaluation activities. Observes and responds to individual emergency situations, and sets priorities for care based on individual acuity, preferences, and the possibility of effective intervention. Implements nursing interventions in response to changes in individual status and unusual circumstances. Interprets assessment data to determine if individuals are at physiological or psychosocial risk. Takes steps to decrease stress and increase the effectiveness of individual coping mechanisms; uses effective communication strategies with individuals, families, staff, and the general public. Assesses individual status upon entry to the hospital and throughout hospitalization. Identifies implications of individuals’ statuses and prioritizes data collection based on individuals’ immediate needs. Preceptor: Guides lower-level, and new to specialty, registered nurses in choosing nursing interventions, developing skills, and applying knowledge. Serves as a resource nurse for staff members, orienting new staff and providing feedback on nursing skills for specialty care areas. Develops and maintains care standards knowledge bases for specialty care areas and develops care guidelines for unit-specific individual populations. Maintains a minimum standard of continuing education for specialty areas. Evaluates and assesses individual staff and nursing student development needs. Counsels and implements a variety of teaching strategies to meet those needs in consultation with nursing management staff. Participates in in-service and other educational programs related to clinical specialties. May identify, develop, and implement nursing research studies on clinical units, in consultation with medical staff consultants. Clinical Management: Consults and coordinates with other health care team members to resolve complex clinical issues and develop short and long-term clinical goals for assigned units. Integrates and evaluates clinical care knowledge specific to the needs of unit individuals; develops unit-specific policies and procedures in consultation with nursing management staff. Identifies and implements corrective actions to improve the quality of individual care; may oversee equipment testing and supplies. Leads and attends various hospital committees. Assists Registered Nurse Supervisor 1 (T&RC) staff with implementing and achieving unit operating changes; may take charge of a unit in the absence of a Registered Nurse Supervisor 1 (T&RC). Resolves individual relation issues. Identifies and reports issues related to individual care, operation, and quality outcomes. Identifies potential conflict situations and facilitates solutions. Registered Nurse Supervisor 1 (T&RC) Plans, directs, and supervises nursing services on an assigned unit, including direct supervision of Registered Nurse 1 (T&RC) and other staff. Directs the development and implementation of nursing care plans on a unit. Provides individual care, and functions as an educator and nursing services expert. Assesses work methods of nursing staff and provides feedback to improve the efficiency and effectiveness of care delivered. Coordinates medical and nursing care needs with other health care providers and departments. Ensures proper functioning of equipment and unit compliance with safety and environmental standards. Monitors nursing care quality standards, and implements corrective actions as needed. Attends and participates in medical committees to represent nursing staff needs and the individual population, and participates in writing of care standards. Identifies staff training needs and collaborates with unit educators to develop and implement educational programs. Develops and implements orientation plans for new employees and participates in staffing decisions.
