Proactively identify recently discharged inpatient, observation and emergency department patients and coordinate timely post-discharge follow-up in alignment with TOC and Transitional Care Management (TCM) requirements, with the aim of addressing root causes of utilization and supporting patients to prevent avoidable readmissions or return visits. Conduct targeted patient and provider outreach via phone, telehealth and in-clinic visits to close care opportunities, provide tailored education on preventive care, chronic disease management, and medication management. Conduct post-discharge outreach to assess understanding of discharge instructions, bottles-out medication reconciliation, symptom monitoring, and follow-up appointment adherence. Identify and escalate barriers, collaborating with providers and care team to prevent readmissions and avoidable ED utilization. Collaborate effectively with interdisciplinary teams, including providers, care assistants, center administrators, medical assistants, pharmacy, and quality improvement staff—to implement evidence-based interventions and optimize workflows. Document all outreach efforts, clinical interactions, and outcomes accurately and in compliance with organizational and CMS regulatory standards. Prepare, participate and discuss patients in center huddles and high-risk rounds with providers and the center-based and interdisciplinary team. Participate in quality improvement projects, provider education sessions, team huddles to stay current with evolving clinical guidelines and organizational priorities. Monitor progress toward Stars and Transitional Care Management goals, proactively identify barriers, and help develop innovative solutions to improve clinical performance and patient engagement. Support clinic operations through provider collaboration, care coordination, and community education initiatives. Coordination and facilitation of center and market-based Wellness Events-focused in-person engagement for Stars care opportunity closures. Maintain patient confidentiality in accordance with HIPAA. Document patient encounters accurately and timely in the indicated platform (e.g., medical record). Follow organizational policies related to safety, infection control, and attendance. QUALIFICATIONS: Must be at least 21 years of age (some exceptions may apply) Must have at least 1 year of caregiving experience with the senior population Must have sufficient communication and language skills to effectively perform duties RESPONSIBILITIES: Execute client care plans and document daily activities Assist clients with activities of daily living Provide clients with household assistance: food preparation, monitoring home safety, laundering, light housekeeping Provide clients with personal care assistance: hygienic care, mobility assistance, medication assistance, companionship REQUIREMENTS: Schedule: Consistent and flexible shifts (WEEKENDS ONLY) Experience: One year of formal caregiving experience in senior care WORKING ENVIRONMENT: Agency office Client homes Skilled, Independent and Assisted Living Communities JOB RELATIONSHIPS: Supervisor: Executive Administrator PHYSICAL REQUIREMENTS: Able to lift 50 lbs. using safe, ergonomic movements Able to stand, walk, reach, crouch, use hands and fingers Able to talk, hear and see Physically and mentally able to drive a vehicle COMPENSATION: Job Type: Part-time Pay: Starting at $15-$17/hr, — pay range reflects caregiver experience and client care requirements. Job Summary: The Clinical Resource Nurse (CRN) assesses, plans, develops, implements and evaluates education activities, competency validation, clinical care, programs and initiatives for the assigned department/units. The CRN demonstrates expert clinical knowledge and skills and effective teaching/education skills and supports system level nursing clinical and education nursing initiatives. The CRN provides leadership within the department including: quality improvement activities; supports effective human resources management; and supports resource accountability. The CRN exemplifies professionalism and promotes a customer-friendly environment by utilizing ServiceFIRST behaviors in interactions with patients, families, and staff members. The CRN is responsible for facilitating an environment that cultivates exemplary patient care. As a member of the nursing leadership team, the CRN establishes a setting that supports professional nursing practice and empowers nurses to provide safe, effective, compassionate and efficient nursing care in collaboration with all health care disciplines. The CRN facilitates enculturation of the Harris Health System Nursing Professional Practice Model (NPPM), its goals and achievement of targeted outcomes at the unit level. The CRN supports compliance with standards established by professional nursing organizations; national, state, and local regulatory agencies and institutional policies and procedures. The CRN holds high standards of integrity and ethical behavior, mentors others, role models professionalism, and participates in activities such as nursing organizations, nursing research, and the community. Minimum Qualifications Degrees: Graduation from an accredited School of Nursing with a Master's degree in Nursing or related healthcare field is required. Licenses and Certification: Licensed to practice nursing in the State of Texas. Related specialty certification or certification as a nursing professional development specialist (NPD-BC) required within two years of employment. Basic Life Support from a hospital-based American Heart Association (AHA) approved program. Other certificates as required by unit/service. Work Experience: Five (5) years of work experience in related clinical area of practice is required. Management/Experience: Two (years) related clinical/leadership experience is required.
