Responsibilities Key responsibilities consist of assisting patients with ADLs, including bathing, grooming, oral care, toileting, and feeding. Providing mobility support including repositioning, transfers, and ambulation using proper body mechanics and assistive devices. Obtaining and documentation of vital signs (temperature, pulse, respirations blood pressure, oxygen saturation) per protocol. Monitor and report changes in patient condition to RN promptly. Assist patients with elimination needs, including use of bedpans, urinals, condom catheters, pure wicks, and provide catheter care (per scope of practice). Documentation of accurate intake and output per protocol. Maintain a safe patient environment by adhering to fall precautions, bed alarms, and safety protocols. Follow standard and transmission -based precautions per hospital guidelines. Perform proper hand hygiene and use of personal protective equipment (PPE). Assist in prevention of hospital-acquired conditions (e.g., pressure injuries, falls). Accurately document care provided in the electronic medical record (EMR) in a timely manner. Communicate effectively with nurses, physicians, and interdisciplinary team members. Participate in bedside shift reports and patient rounding.
