Responsibilities The primary purpose of the Staff Nurse (Per Diem) position is to work on an as needed basis and be accountable for the care activities, safety and goal achievements of assigned patients through assessing, diagnosing, planning and intervening in actual or potential health problems and evaluating patient’s response to the care. These responsibilities also include patient education and discharge planning. The Registered Professional Nurse places emphasis on mobilizing patients and preventing skin break down, falls and implements the appropriate measures to achieve this. The Registered Professional Nurse is a member of a designated care team and collaborates, coordinates and guides team members in planning patient care in a safe environment and achieving patient care goals according to the patient/family developmental stage and cultural expectations. Responsibilities The primary purpose of the Advanced Practice Nurse (APN) position is to provide high quality patient care. The APN utilizes advanced practice nursing expertise to provide care to patients in the Emergency Department. Overview About the Role The Oncology Nurse Navigator (ONN) works with cancer patients and their families to remove barriers to care, provide emotional support and resolve issues related to the cycle of cancer care. The ONN works as a liaison to and in collaboration with the patient, physician, and other healthcare providers to manage the complex needs of cancer patients. She/he serves as an advocate for the patient and works to enhance the patient experience, breaking down barriers so the patient receives timely and compassionate care. The ONN is involved in community outreach and education and serves as an expert in their clinical area of focus. She/he will serve as a tumor board coordinator for their assigned disease focus and provide clinical care as directed for their assigned patient population. Additionally, the ONN is proficient in quality practices and will track navigation metrics and quality indicators for reporting to Cancer Committee and for accreditation purposes. Serve as a leader on committees and projects. Responsibilities What You’ll Do Serve as a primary resource for cancer patients and their families, helping them navigate the healthcare system and overcome barriers to treatment. Provide emotional support, guidance, and education throughout the cancer care continuum. Collaborate closely with patients, physicians, and interdisciplinary healthcare teams to coordinate comprehensive cancer care. Advocate for patients to ensure timely access to services, treatments, and resources. Enhance the overall patient experience by addressing concerns and facilitating compassionate, patient-centered care. Qualifications What You’ll Bring Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred. Minimum of four (4) years of clinical nursing experience required, preferably in oncology and/or the assigned disease specialty. Current New Jersey Registered Nurse (RN) license required. Oncology Nursing Certification required or must be obtained within 24 months of employment and maintained throughout employment. Knowledge of the healthcare system, cancer care, community resources, and social service resources for cancer patients required. Strong leadership, organizational, written, verbal, and interpersonal communication skills required. Responsibilities POSITION SUMMARY Plan, implement and evaluate nursing care provided, including restorative care for residents in a manner that is reflective the ESCC Core Values. As the direct care provider of health services, he/she shares appropriate information with all other health services staff in an effort to assist each resident in achieving his/her maximum level of development. Directs and supervises CNAs, unit a clerk, LPN and other RN’s when in charge. Establishes effective working relations with staff residents and family members. Ensures that the Center and the Department of Nursing policies and procedures are follow in the planning implementation and evaluation of care. Review medication administration records for completeness at end of shift. Give and receives the nursing report and follow through on any special instructions. When in charge makes periodic checks to ensure that prescribed treatments are being properly administered by LPN’s and CNAs and to evaluate the residents physical and emotional status. Participates in the orientation of new staff residents and families to the unit. Carries out restorative and rehabilitative programs including teaching resident to wash/feed/dress self as appropriate. Ensures that the facility policy on the use of restraints is followed and that documentation is timely and accurate. Ensure that the unit is kept clean free from clutter and that the supply room door is kept locked; request the supervisor’s intervention as necessary to achieve this. Follows infection control standards and ensures that assigned personnel do likewise. Keep abreast of new nursing policies and procedures. Keep nurses notes, plan of care and all other documentation pertaining to your assigned resident timely accurate and complete. Carries out end of shift responsibility e.g. supplies are available for next shift’. Unit is clean and tidy; all documentation is complete and accurate. Other related duties as may become necessary or as directed by your supervisor, department director, and/or administrator. Qualifications Education Requirements: Graduate of an accredited RN program. Coordinate and oversee the Care Plan meetings. Responsible for ensuring completion and follow up of occurrence reports, medication error forms, skin care forms and continually work on reducing occurrences and medication errors through root cause analysis, education and quality improvement processes. Rounds on Neighborhoods for the purpose of obtaining clinical information on residents, to support and educate staff, to assist with family issues and endorses complete information to next shift. Promotes 24 x7 accurate, objective clinical and administrative reports. Responsible for review of EMR status board and other variance reports and follow up with staff on outstanding items. Establish a schedule for and coordinate neighborhood meetings. Assure that information is disseminated to all appropriate parties timely. Overview About the Role The Advanced Practice Nurse (APN) is a skilled health provider who through a written collaborative practice agreement with a physician is credentialed through the Medical Staff Office. The APN in the Surgery Practices utilizes critical judgment in the performance of comprehensive health assessments, differential diagnosis, and the prescribing of pharmacologic and non-pharmacologic treatments in the direct management of patients diagnosed with a variety of surgical conditions/diseases.
