Role Overview We’re seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across our partner communities. This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live. What You’ll Do Routine & urgent assessments; chronic disease management Post-discharge follow-ups, med reconciliation, labs/diagnostics Coordinate with facility staff, specialists, families/POAs Same-day documentation; CMS/regulatory compliance Schedule & Setting Full-time position with a structured weekday schedule. You'll spend 3 days delivering on-site care with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are in-office at our Irvine headquarters for administrative and documentation tasks. Description Under the general supervision of the Admin Nurse III and the Senior LVN Supervisor, the Senior Licensed Vocation Nurse provides nursing support and leadership in the Department of Urology. Major duties include: provide patient care in an outpatient setting, assist with urologic procedures such as cystoscopies and bladder installations, start I.V.s, perform venipuncture, administer medications, manage patient flow, collect data as appropriate and provide back-up for the clinic front desk, as needed. The incumbent spends a significant portion of time contributing to the development and implementation of a teaching plan related to self-care for the patient. The Sr LVN will travel and work at other UCLA Urology clinics throughout the community as assigned by the supervisor for coverage. Other duties as assigned. Responsible for oversight of and/or scheduling, payroll and other administrative duties Assures that all initial assessments and required reassessments are conducted by qualified team members within required time frames, including documentation completion, submission and /or synchronization. Ensures that all patient/family services comply with federal, state(s) and local regulatory requirements and with accepted professional standards. Ensures protocols are utilized to guide operations and promote adherence to regulatory and internal process requirements. Monitor expenses related to patient/family services to ensure budgetary compliance in collaboration with IDG and Site Leadership. Serve as a resource person and liaison for the Social Worker, Volunteer, Music Therapy and Bereavement Departments.
