Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Recruits, orients, trains, coaches, counsels, mentors, disciplines, and evaluates staff in accordance with all internal policies and procedures. Communicates values, strategies, and objectives of department or division on a regular basis. Assigns accountabilities, delegates tasks and responsibilities, and plans, monitors and appraises job performance. 3. Develops, maintains and reconciles departmental budget with financial responsibility and oversight for department or division. 4. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes, or recommends changes to superiors as necessary. 5. In conjunction with Nursing, Medical and Quality Leadership, coordinates educational and performance improvement activities within a designated clinical specialty. 6. Develops and designs of action plans to support clinical performance and competency and provides support and educational resources to departments and services relative to nursing care delivery within the context quality assessment and improvement processes. 7. Establishes and implements short and long term goals, objectives, policies and operating procedures for high level of performance in nursing care delivery. 8. Coordinates and leads in the execution of day to day educational program activities, as appropriate to program objectives and areas of clinical expertise. 9. Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence. 10. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Master's Degree in Nursing is required. Masters in Nursing or health related field is preferred. NP Certification in specialty area preferred. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Certification in specialty area preferred. Description Patient Care Associate MSH GP9C Med/Surg PT (0.8) 4 shifts/wk Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Description The Nurse Manager (NM) is responsible for the management of nursing practice and operations on a designated unit/service setting such as inpatient, procedural, emergency department or ambulatory care. Responsibilities PATIENT CARE 1. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of patient experience. 2. Oversees patient care delivery and daily operations across assigned inpatient units and/or ambulatory practice areas, utilizing Leader Standard Work to ensure consistency in leadership practices, operational excellence, patient access, quality outcomes, and the implementation of evidence-based best practices. 5. Works collaboratively with the Center-led Staffing Department as needed to ensure staffing grids are met (as covered by this service) and optimize workforce deployment. 6. Support labor management strategies by collaboratively maximizing retention strategies and internal resource utilization and minimizing reliance on premium labor, verifying that staffing plans maintain quality, safety, and appropriate clinical expertise. 7. Monitors staffing trends and workforce metrics, validating and implementing data-driven recommendations to support informed staffing decisions and continuous workforce improvement. 8. Works collaboratively with physician dyads and all members of the healthcare team to promote patient care and regulatory standards. 9. Prioritizes recruitment and retention strategies in accordance with system and local hospital retention goals. 10. Implements and monitors unit salary and supply budgets, including capital equipment allocation according to MSHS financial goals. 11. Monitors availability and maintenance of nursing equipment and supplies 12. Maintains position control of established positions and manages the recruitment and retention of qualified staff. 13. Performs disciplinary actions as necessary with collaboration with Nursing and Labor Relations. 20. Acts as a management witness in disciplinary activities. 21. Ensures all staff have met their orientation goals/weekly progress reports and annual competency requirements and participates in the Competency Planning Tool for unit/service. 22. Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with all hospital policies and procedures. Maintains records of minutes for staff team meetings. 23. Encourages, recognizes and rewards staff participation in quality and safety initiatives, compliance and successes. 24. Key Responsibilities: - Assist clients with daily living activities, including bathing, dressing, grooming, and meal preparation. - Provide companionship and engage clients in meaningful activities. - Perform light housekeeping tasks to maintain a clean and safe environment. - Monitor and report any changes in clients' health status to the care coordinator. - Follow individualized care plans and provide care with compassion and respect. - Ensure clients' comfort and safety at all times. - Maintain accurate and timely documentation of care provided. Qualifications: - HHA certification in the state of New Jersey. - Previous experience in home care or a related field is preferred. - Strong interpersonal and communication skills. - Compassionate, reliable, and dedicated to providing excellent care. - Ability to work independently and as part of a team. - Valid driver's license and reliable transportation are preferred.
