Job Summary Responsible for planning and providing nursing care to patients according to AORN Standards of Nursing Practice and Hospital policies and procedures. Duties & Responsibilities Utilize the nursing process to implement goal-directed nursing care for a defined group of patients. Monitor medical condition of assigned patients at all times and report to Nursing Supervisor and Physician or designee any patient a=care problems, changes in condition or unexpected occurrences. Assess patient’s healthcare status based upon physical examination, related reports and records and consultation with appropriate personnel. Identify priorities and provide immediate intervention on admission based on intraoperative report and physical assessment. Educate patients/family regarding condition, treatment, discharge/transfer planning and follow up examinations if needed. Seek feedback from patients/family to evaluate understanding and document. Perform a variety of nursing skills in compliance with physician orders and established Hospital policies and procedures. Communicate with physicians and other departments to consult on patient status and to ensure continuity of care. Maintain patient’s medical records in an orderly manner and document all care provided and significant information. , legibly, accurately and completely. Respond to cases of cardiopulmonary arrest, fire, disaster or other emergencies in accordance with established procedures. Assist in the orientation of new employees. Provide direct patient care, including examination and treatment under an approved protocol. Evaluate outcomes and adjust nursing care processes as needed to ensure optimal patient care. Assume charge nurse responsibilities when necessary. Assume responsibility for own professional development, setting goals and striving to achieve them through available resources, such as attending seminars, in-services and/or reading journals. Performs all other related duties as assigned. Participate in quality assurance activities. Document issues and communicate/consult with the Nurse Manager Requirements Graduation from an accredited nursing program and a minimum of three years job-related experience or equivalent is required. RI State Licensure is also required. Experience must include one year critical care nursing, PACU experience preferred. BLS and ACLS required. Job Summary The Nurse Navigator functions on the multidisciplinary team as an advocate, interpreter, educator, and counselor for orthopedic surgical patients. Is responsible for ensuring all adult patients with an oncology diagnosis receive quality and comprehensive services, including coordinating patient care throughout the continuum in collaboration with the multidisciplinary team. The Nurse Navigator will serve as a clinical resource with expertise in orthopedic care management and will serve as a liaison throughout the facility and in the community regarding services provided for this unique patient population and will provide expert nursing care, which includes: direct clinical practice, consultation and education. Duties & Responsibilities Demonstrates the knowledge, skill, and coordination to provide nursing care and guidance to the cancer patient from screening to survivorship. Systematically and continually performs the functions of assessing, planning, implementing, and evaluating the care according to the nursing process and Nursing Society Standards of Practice. Provides education and information to the patients and their family, helping to make the care seamless, continuous and comprehensive. Initiates and documents patient teaching including family and significant others based on assessment of needs, including psychosocial, and lifestyle practices. Responds to patient request for information regarding the disease process, expected side effects of treatment and community resources. Uses appropriate patient Education documentation modality. Supports the patient during difficult decision-making periods. Assists in coordination of end of life care for patient and family and provides emotional support. Functions in an organized and time conscious manner. The Navigator partners with patients, families, interdisciplinary team, and community resources to provide well-coordinated, timely, compassionate, exemplary, interdisciplinary care and communicates with all members of the healthcare team, as appropriate about patient/family needs and concerns. Initiates and performs ongoing review of policies related to service provided and when appropriate, updates or writes new policies to enhance professional practice. Serves as a resource for community educational events, such as health fairs, screenings, symposiums, and lectures as well as support for staff education. Preserves and protects patient rights to confidentiality. Performs other related duties as requested or directed. Requirements Bachelor of Science in Nursing required with at least 5 years of nursing experience. Nurse Navigator Experience Required. Must have and maintain current licensure as a Registered Nurse (RN) in Rhode Island and CPR certification. Orthopedic specialty nurse certification is preferred. Nurse Navigation Certification Preferred and/or obtained within 6 months of hire.
