Weekend Clinical Support & Response • Serve as a visible nursing presence across assigned residential programs throughout the weekend • Respond to clinical concerns and provide nursing assessments and interventions within scope of practice • Provide consultation and guidance to residential staff regarding illness, injury, medication concerns, and changes in condition • Triage healthcare concerns and coordinate with healthcare providers, pharmacies, hospitals, and leadership as needed • Respond to urgent clinical calls and support staff with time-sensitive healthcare issues • Escalate significant clinical concerns to the Director of Integrated Health Services and operational leadership • Provide support during unexpected nursing needs or staffing shortages • Travel between residential programs based on clinical priorities and organizational needs Admissions & Transitions of Care • Support weekend admissions, hospital discharges, emergency department returns, and other transitions of care • Review physician orders, medications, treatment plans, and discharge documentation • Coordinate with pharmacies, healthcare providers, nursing leadership, and residential teams to ensure continuity of care • Identify and address medication discrepancies, missing orders, or other concerns affecting safe transitions Program Rounds & Quality Monitoring • Conduct clinical rounds of healthcare plans and physician orders • Provide immediate coaching and corrective guidance when concerns are identified • Communicate trends and significant findings to nursing and operational leadership Medication Administration & MAP Compliance • Support compliance with Massachusetts Medication Administration Program (MAP) requirements • Conduct medication audits and medication administration observations • Review Medication Administration Records (MARs), physician orders, controlled medication documentation, and related records • Assist staff with medication-related licensing, accreditation, and BAMSI healthcare standards • Identify medication, documentation, infection control, safety, and clinical risks • Support corrective actions and continuous quality improvement efforts • Maintain infection prevention standards, safety practices, and universal precautions Staff Support & Education • Serve as a mentor and clinical resource for nurses and development of newer nursing staff • Help ensure consistent clinical practices across multiple programs What You Bring: Licensure • Current Massachusetts Registered Nurse (RN) or Licensed Practical HHA certificate Job Responsibilities: Enables patients to stay in their homes by monitoring and recording patient condition, providing support and personal services, and teaching families. Monitors patient condition by observing physical and mental condition, intake and output, and exercise. Supports patients by providing housekeeping and laundry services, shopping for food and other household requirements, preparing and serving meals and snacks, and running errands. Assists patients by providing personal services, such as, bathing, dressing, and grooming. Helps patients care for themselves by teaching use of cane or walker, special utensils to eat, special techniques and equipment for personal hygiene. Helps family members care for the patient by teaching appropriate ways to lift, turn, and re-position the patient. Advises on nutrition, cleanliness, and housekeeping. Records patient information by making entries in the patient journal and notifying nursing supervisor of changing or unusual conditions. Maintains a safe, secure, and healthy patient environment by following asepsis standards and procedures, maintaining security precautions, and following prescribed dietary requirements and nutrition standards. Protects the home care agency by adhering to professional standards, home care policies and procedures, federal, state, and local requirements. Making frequent rounds on nursing unit(s) to ensure that all personnel are performing their work assignments in accordance with established nursing standards, and that all resident’s needs are being met 7. Monitors documentation procedures of nursing staff. Follows- up on problem areas with employees and makes oral/written reports to the Director of Nursing 8. Any other duties deemed necessary by the Director of Nurse Personnel Functions 1. Follows work assignments, and schedules to complete assigned tasks. 2. Performs all assigned tasks in accordance with established policies and procedures. 3. Cooperates with interdepartmental personnel, as well as other departments within the facility to assure that services are properly maintained to meet the needs of the residents. 4. Adheres to the rules of attendance as outlined and received on orientation 5. Recognizes the needs and works to facilitate appropriate assignment of staff within the frame work of established staffing patterns. 6. Meets with staff on regularly scheduled basis to assist in identifying and correcting problem areas, and/ or improvement services. 7. Functions in the role of staff nurse or provides direct care to meet the needs of the residents as needed/ required. 8.
