Overview Starting Rate: $33.00/hour Schedule: Monday through Friday (during business hours between 7a to 5p, some flexibility offered) The Licensed Practical Nurse provides nursing services as defined in the LPN scope of practice and under the direction of the Residential Program Nurse. Minimum Education Required High School Diploma/GED Shift First Shift Responsibilities Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments and treatments. Provide emergency first aid in the case of a medical crisis. Coordinate with residents, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment or medication. Implement, oversee, document and supervise medication management in accordance with agency and funder regulations. Oversee direct care staff, under the direction of the Residential Program Nurse. Monitor and evaluate the continuity and level of care provided, devising ideas for quality improvement when needed. Participate in staff meetings, trainings, service planning meetings, resident and family meetings. Qualifications Massachusetts as a Licensed Practical Nurse and 1 year experience in a community healthcare setting. Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff. Excellent written and verbal communication. Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients. Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving culturally diverse populations. Together with the medical provider, the RN Case Manager collaborates with all members of the care team, focusing on the delivery of efficient, high-quality care. This position ensures the appropriate utilization of clinical resources with a goal of a safe and timely discharge for the patient. This role navigates health system services to support effective transitions while advising the team on healthcare industry compliance. The RN Case Manager must be adept at driving throughput metrics, clinical effectiveness, and fiscal responsibility. Job Description: Essential Functions: The RN Case Manager collaborates with the health care team to develop the plan of care and patient flow. Tasks: Reviews all cases within 24 – 48 hours or the next business day of admission/bed placement and each day throughout the stay to facilitate care progression to establish an anticipated length of stay and transition planning needs. Collaborates with the medical team to formulate a treatment plan to include care transitions and promote patient flow. Completes an initial assessment of all admissions/observation patients to identify barriers that impact the length of stay and discharge planning. The assessment should also identify the needs of the patients, acknowledge current resources available, and anticipate future resources needed to facilitate successful transitions. Previous phlebotomy experience is strongly preferred. • Maintain appropriate inventory of immunizations and monitor expiration dates. • Administer immunizations, as applicable, when appropriately certified or when the employee meets established experience requirements and is working under the supervision of a qualified provider. • Support departmental quality and safety initiatives, including adherence to patient identification requirements, hand hygiene, infection prevention practices, and other established clinical standards. • Maintain a professional, patient-centered approach when interacting with patients and families, responding to questions and concerns and escalating clinical concerns to the appropriate nursing or provider staff. • Perform other administrative and clinical support duties as assigned based on departmental needs, training, and competency.
