Job Details St. Francis Hospital Critical Care Advanced Practice Provider (APP) is a valuable team member. Primary responsibility is the management of critically ill and complex (multi-organ) patients in an intensive care setting. This setting includes the management of patients in the Cardiothoracic ICU (CTICU), Surgical ICU (SICU), Medical ICU (MICU) and Intermediate Care Unit (IMCU). The APP is under the direct supervision of an in-hospital 24/7 Critical Care board certified Intensivist program. The APP performs physical exams, collects and documents data, conducts approved diagnostic and therapeutic procedures, orders and schedules laboratory studies and professional consultations, prescribes appropriate interventions and medications, and provides direct patient care services in a specific ICU. Responsibilities: Provide direct clinical services for critically ill adult patients with multi-system disease /failure, according to established protocols and evidence-based clinical practice standards. Obtain patient health histories and complete physical examinations. Document findings, noting pertinent normal and abnormal findings. Order and interpret appropriate diagnostic and laboratory tests. Explain the necessity, preparation, nature and anticipated effects of procedure(s) to patients, patient's family, staff, and healthcare interdisciplinary team members. Perform approved therapeutic or diagnostic procedures based upon patient's clinical status. Formulate and implement a treatment plan for acute illness management in collaboration with the designated primary supervising physician and/or the intensivist. Utilize prescriptive privileges as allowed by NY licensure. Assess patient for change in status and institute appropriate interventions. Initiate necessary emergency interventions to stabilize patient. Participate in our Clinical Education and Science Simulation center for onboarding and strengthening of both didactic and procedural skill sets. Develop procedural skill sets and continued competency including placement of central lines, pulmonary artery catheters, hemodialysis catheters, trans-venous pacing wires, arterial lines, PICC lines and mid lines. Other complex management includes mechanical support devices such as Impella, Left Ventricular Assist Devices (LVAD), and Extracorporeal Membrane Oxygenation (ECMO). Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. • Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. • Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. • Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. • Participates in special projects and performs other duties as assigned. Description The Assistant Nurse Manager of ID (Infectious Disease) Transplant / Ophthalmology / Ambulatory Float Pool is a Registered Professional Nurse with broad knowledge and skills who assumes the role of a clinical leader in the daily functioning of the unit and oversight of quality patient care delivery, in conjunction with, or in the absence of, the Nurse Manager. Essential Duties and Responsibilities: Provides direct supervision and monitoring of the nursing care given to our residents on a daily basis ensuring required documentation is completed. Provides leadership and guidance to the nursing staff. Provides treatments and medications for our residents. Attends and participates in “at risk” meetings, morning clinical meetings, QAPI meetings, safety meetings, and care plan meetings. Shares “on-call” responsibilities with other supervisory staff and provides direct care as necessary. Investigates accidents and incidents related to our residents and staff. Writes and reviews care plans identifying the problem/needs of each resident, the care to be given, goals to be accomplished and updating the care plan as needed. Conducts nursing assessments. Reviews resident change in condition report daily to ensure follow up occurs including appropriate interventions are implemented and the family has been notified. Complies with and ensures that staff is in compliance with HIPAA rules and regulations. Initiates disciplinary action as necessary for staff members. Participates in annual survey and/or complaint survey processes as directed. Performs additional duties as assigned.
