Essential Job Functions and Responsibilities: Provides direct care to patients under direction of an RN, based on current policies and procedures, accepted standards of care, and established competencies Under the supervision of the registered nurse. Observes physical/emotional status of assigned patient/family. Able to prioritize patient care and unit activities. Provides care according to current policies and procedures and established standards of care. Appropriately seeks out assistance to assess patients or render patient care. Is aware of limitations of behavioral health associate and functions within those limits. Provides for the patient/family needs for psychological, emotional or spiritual support in an unhurried, empathetic manner. Meets standards of unit-based competencies on an annual basis. Assumes role of patient advocate. Consistently refers to the critical pathway when rendering care. Participates in patient activities/groups under the direction of the licensed nurse. Assists with emergency code situations according to policy and procedure. Expedites patient care needs by answering call lights, intercom, verbal requests, or telephones promptly. Documents all patient care appropriately All documentation is concise, clear, pertinent, accurate and timely. Documents safety measures, actions, and/or communications with patient/family. Communicates effectively with all healthcare team members in the implementation of patient care Works with colleagues and other disciplines in a spirit of teamwork, professionalism and goal attainment. Seeks direction from other healthcare team members when necessary in order to clarify a patient's need or condition. Consistently shares all pertinent observations and recommendations regarding patient needs to the registered nurse or case manager. Consistently communicates patient responses to the critical pathway to the RN or case manager in a timely manner. Participates in Multidisciplinary Team Meetings when appropriate. Acts as a resource person Responds to requests form peers in acceptable time period. Participates on committees. Assists the Registered Nurse in the orientation of new employees. Assesses clinical environment and recommends appropriate changes/improvements Identifies and communicates areas for improvement in clinical environment. Educational/Minimum requirements: Must have High School Diploma or equivalent (GED). Must have a valid CPR w/BLS certification from the American Heart Association. Must have a minimum of 1 year of experience working in a long term care and/or behavioral health setting. State/Federal Mandated Licensure or Certification Requirements: Must have a valid CPR w/BLS certification from the American Heart Association. Must have a valid and active CNA Certification in the State of Connecticut. Bristol Hospital Mandated Educational Requirements: General orientation at time of hire. Accu Chek, CPR, Fire/Safety/Infection Control annually. Healthcare Provider CPR from American Heart Association or Professional Rescuer, CPR by American Red Cross or other similar programs deemed appropriate by management. Additional cross-training as appropriate to perform diverse job duties. Successfully complete all training programs as determined by the Hospital. Other unit based competencies/skills as required. Special Requirements: Knowledge of basic patient care procedures and medical terminology. Ability to work as part of a team. Data entry and documentation skills. Experience working with a psychiatric population. Physical Requirements: Significant walking, bending, stooping, patient lifting and positioning. Must be able to participate in PERT. Manual dexterity for phlebotomy and data entry. Work Environment: Stressful working conditions in a patient care environment. Exposure to patient fluids and waste. Rapid pace and variable environment. Cognitive Requirements: Mental flexibility to perform diverse duties involved in patient care. Job Summary Urgent care Advanced Practitioners (Physician Assistant/Nurse Practitioner) provide a wide variety of clinical services including treatment of acute episodic conditions such as sore throats, colds, flu-like symptoms, cuts, sprains and minor infections. Functioning in lieu of or alongside of a physician, it is essential for an urgent care Advanced Practitioner to have excellent assessment skills in order to recognize more serious conditions and direct the patient to the emergency room for advanced care when necessary. Under general supervision, assumes 24-hour accountability to plan and provide direct professional nursing care for specified patients throughout the entire episode of service delivery as defined by population and unit. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, CCMC policies & procedures, Joint Commission, and State & federal regulations. Reports to Nurse Manager. CLINICAL PRACTICE - On a daily basis, demonstrates clinical expertise and uses the nursing process to provide safe, appropriate patient care to a defined patient population according to CCMC and unit policies and standards. POPULATION SPECIFIC - Demonstrates knowledge of the differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. UNIT LEADERSHIP - Delegates duties and tasks associated with patient care to clinical staff according to hospital policy and their scope of practice. As assigned, contributes to the leadership of the unit by functioning as the Resource Nurse and/or by participating in the professional development of others through preceptor/mentor activities. Supports the unit/department’s work productivity and resource management activities. DOCUMENTATION/COMMUNICATION - Completes all required documentation in a timely manner, according to CCMC policies and standards; necessary information is effectively communicated to all members of the health care team. PATIENT/FAMILY PARTNERSHIP & EDUCATION - Partners with child and family in evaluating outcomes. Monitors, trends, and evaluates patient/family responses to treatment. Provides appropriate education to patients and families regarding all aspects of care according to CCMC policies and standards; supports the organization’s efforts to patient/family education. CCMC STANDARDS - Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CCMC Standards. Education and/or Experience Required: A minimum of ASN/ADN required upon hire, BSN preferred. If ASN/ADN, must acquire BSN within 3 years of hire. Minimum of 2 years recent perioperative experience. License and/or Certifications Required: Current State of Connecticut Registered Nurse Licensure required or nurse licensure compact (NLC) and maintained thereafter. Current Basic Life Support (BLS) / CPR in adult, child & infant is required upon hire.+ Advanced Cardiac Life Support (ACLS) is required within six months of hire. Specialty licensure/certification may be required, as specified by department. Licenses and/or Certifications Preferred: Certification in OR nursing is preferred. Pediatric Advanced Life Support (PALS) required within 6 months of hire or before staff has completed dept. orientation if this is longer than 6 months. Describe the underlying pathophysiology of commonly encountered disease processes. Describe the physiology associated with various anesthesia techniques and surgical or procedural interventions. Develop an individualized perianesthesia plan of care by implementing: C ommunicating the patient’s current status throughout the perianesthesia continuum of care. Evaluating the patient’s past medical history and surgical plan using the PAT anesthesia protocols Interviewing pre-operative patients to gather necessary data to assist for admission Reviewing all preoperative test results and notifying physicians when results are outside normal limits or incomplete. Serving as initial and primary contact for referral sources to direct screening and clearance of patients scheduled for, or potentially to be scheduled for procedures. Understanding medications and their actions and educating the patient on which medications need to be taken day of surgery. Demonstrating safe operation of equipment and machinery and follows procedures for reporting and correcting an unsafe situation. Speaking up with safety concerns acting as the patient’s advocate. Handling specimens according to facility policy and procedures. Adhering to standard precautions including the use of personal protective equipment. Qualifications Graduate of an Accredited School of Nursing. One year of pre-op, PACU , and/or medical / surgical nursing experience required. Current state licensure to practice within good standings. Successful completion of Basic Life Support (BLS) or Basic Cardiac Life Support (BCLS) within 90 days of employment or documentation of current BLS/BCLS certification. Successful completion of Advanced Cardiovascular Life Support (ACLS) Course within 90 days of employment or documentation of current ACLS certification. Successful completion of Pediatric Advanced Life Support (PALS) course is required when pediatric care is included in the facility scope services within 90 days of employment or documentation of current PALS certification.
