Your Role on the Team The position of Nurse Practitioner Per Diem (43%) in the Department of Medicine, Division of Hospital Medicine & Palliative Medicine, requires a highly competent, independent yet team-oriented individual who will be immersed in Hospital Medicine. Under the general supervision of Alpesh Amin, MD and guided by the applicable policies and procedures, the Nurse Practitioner will participate in the comprehensive care of patients in the ambulatory or inpatient settings and provide multi-disciplinary care coordination, treatment, education, monitoring, and communication with patients and referring physicians. A Nurse Practitioner is an Advanced Practice Provider (APP) who possesses the advanced education, knowledge, clinical skills, and experience necessary to provide advanced nursing care in their area of specialization. This Position May Perform Functions as appropriate to their “scope of practice” (In general, the “scope of practice” includes the management of chronic stable medical conditions, routine care, acute or critical care, and medical and surgical procedures per their approved standardized procedure). Familiarity with state statute for their licenses is necessary. Approved privileges are unique to each person and are unlikely to include ALL privileges. The NP professional may provide patient care in age groups based on educational preparation and at all Health licensed settings and affiliates. The NP has responsibility for the standards of care for their specified patient population and evidence self-direction and accountability both to quality of care and in the development of their role within the all Health locations and affiliates. Advanced Practice Practitioners (APP) are required to be credentialed to practice and be privileged through Medical Staff to function in their clinical role. The Wound Care RN assesses the skin of patients in the facility, monitors the status of wounds, implements specialized dressing change recommendations based on physician orders and clinical expertise, implements offloading and HAPI prevention orders, adapts to changing policy and procedures according to corporate policy. The Wound Care RN provides ongoing evidence based on relevant education to all facility bedside staff related to HAPI prevention, dressing changes, and care of skin. The Wound Care RN will establish and maintain monthly wound care committee meetings which will include relevant participants from their facility. The wound care RN will collaborate respectfully with all departments to create effective wound care practices based on evidence based best practice guidelines and corporate policies. Communicate with physicians / charge nurse / co – workers, as appropriate regarding changes in the patient’s treatment, clinical condition including results of diagnostic studies. The Wound Care RN will report HAPI occurrence to leadership in a timely manner and has the ability to respond quickly and accurately to changes in condition or response to treatment. Participate in team development calls and meetings. Qualifications EDUCATION, EXPERIENCE, TRAINING Must have current and valid license as a Registered Nurse issued by the state. BLS upon hire and maintain current. Previous Acute Care hospital experience preferred. Previous Wound care knowledge and or experience preferred (1 Year). Tuition Reimbursement: Repays up to $600 per calendar year for fees expended to attend a job-related seminar, licensure renewal, workshop, conference, individual course, or other training programs. Tuition Loan: Provides up to $2,000 per calendar year for approved formal certificate or degree programs including prerequisites for approved degree programs. A tuition loan and reimbursement may, with appropriate approvals, be combined for a maximum of $2,600. Critical Pipeline Loan: Tuition loans up to $5,250 for specific degree programs at approved, accredited schools identified by the organization, including BSN and MSN. Employees may have their loans Forgiven if they continue eligible employment for 12 months after completion of the term for which the loan was given Our Units: The Float Pool is a high level performing department staffed with healthcare professionals who are equipped to work in various areas of the hospital. These highly trained professionals working in the Float Pool support the organization by having the ability to care for some or all of the specialty patient populations. The organization has several specialty units that the float pool supports which include: Intensive Care Unit (ICU), Emergency Department (ED), Post-Anesthesia Care Unit (PACU), Progressive Care Unit (PCU), and four Medical-Surgical/Telemetry units (MS/TELE). Our Intensive Care Unit is a 12 bed Medical and Respiratory MICU caring for patients with acute medical issues such as sepsis, GI bleeding, post code blue and respiratory issues. On this unit, our registered nurses typically have a 1:2 ratio and 1:1 ratio patients that meet the criteria. Each room has a HEPA filter and PAPRS readily available. Our Emergency Department (ED) has 20 patient beds and sees on average 100 people a day. There are Registered Nurses, Break RNs, Charge Nurses, Physicians, Physicians Assistants, and Nurse Practitioners on this cohesive multidisciplinary team. Our emergency medicine specialists are dedicated to providing comprehensive, leading-edge emergency care to a diverse population. Position Summary: The Registered Nurse assumes accountability and responsibility for the delivery of patient care in the hospital setting and for the respective patient population. Provides direct and indirect patient care services, including the administration of medications and therapeutic agents necessary to implement treatment, disease prevention, or rehabilitative plan of care. Demonstrates advanced assessment skills to evaluate the signs, symptoms, reactions, behaviors, or general appearance to determine normal versus abnormal characteristics. Displays knowledge and is able to identify and make special adjustments to the specific populations needs, including cultural, spiritual, age, psychosocial, communication, gender, sexual orientation, economic, education, family and condition needs. Provides assistance to co-workers, new staff, temporary staff and students Functions as a change champion and agent and participates in departmental quality improvement activities Hours: 7:00 p.m. – 7:30 a.m. Nights Qualifications Educational Requirements: Graduate of an accredited nursing program. BSN is preferred Associate's Degree in Nursing required License/Certifications: Valid California RN license BLS through the American Heart Association or American Red Cross ACLS certification through the American Heart Association or American Red Cross PALS if floating to the Emergency Department Experience: A minimum of 2 years of acute care nursing experience required Demonstrated dedication to customer service and ability to meet the needs and expectations of patients and health care colleagues
