What You Will Do: The RN Case Manager works collaboratively with patients, social workers, physicians, other practitioners, caregivers and community agencies in coordinating and overseeing the interdisciplinary plan of care for a patient in a home care/home hospice environment. Assesses, plans, facilitates and evaluates timely coordination of high quality, cost-effective care. Responsible for improving the health of patients dwelling in their own homes. This requires a nurse who is comfortable working independently, enjoys teaching, is a critical thinker and has strong communication skills. The RN Case Manager provides hands on care in the home environment and offers autonomy, flexibility and the luxury of caring for one patient at a time. Ideal candidates also possess negotiation skills along with strong analytical and computer skills. Experience with pre-acute and post-acute care as well as community resources is preferred.
