Assessment and Care Planning: Conduct initial and ongoing assessments of patients' health status. Develop and implement individualized care plans based on these assessments to meet the patient's needs. Wound Care: Provide wound care management, including dressing changes, infection control, and monitoring healing progress. QUALIFICATIONS Graduate of an approved school of nursing or Bachelor’s degree of Nursing from an accredited college/university Currently licensed, in good standing, to practice in New York as a Registered Nurse. Minimum 1 year community health nursing experience or equivalent professional experience. Knowledge of nursing and medical practices, procedures, regulations and guidelines pertaining to home care, including OASIS. Complies with accepted professional standards and practice.
