Summary: 3 East is a 32-Bed unit specializing in cardiac surgery and cardiology medicine. 3 East cares for patients with Acute Cardiovascular disorders frequently requiring remote telemetry monitoring, in a Progressive Care Unit environment. These include, but are not limited, to the following patient groups: Acute MIs, Heart Failure, Pulmonary Edema, Transcatheter Aortic Valve replacement (TAVR) Arrhythmias, Post-open-heart surgery (OHS), post endovascular procedures, post cardiac catheterization patients, s/p pacemaker and ICD insertion, Valvular Heart Disease, and medical/surgical overflow. The Clinical Nurse 2 Colleague is an RN that practices as a full partner on the health care team and consistently demonstrates safe practice within the organizational professional practice model. The Clinical Nurse 2 Colleague possesses the knowledge, skills and the attitude to effectively meet standards for competent professional nursing practice as specified in the Clinical Nurse 2 position description addendum. World’s First Pediatric Hospital-based Birthing Facility : Being located in a pediatric hospital allows the SDU to provide immediate access to pediatric specialists if needed, ensuring a smooth transition for babies who require intensive care or surgery after birth. State-of-the-Art Facilities : The two operating rooms dedicated to cesarean sections and fetal surgeries help accommodate the unique needs of the pregnancies being managed in the SDU. The 15-bed Labor, Delivery, Recovery, and Postpartum (LDRP) rooms provide a nurturing environment for the mothers to labor and recover, while also allowing flexibility for individual care plans. The two infant stabilization rooms ensure that babies born with complications are immediately stabilized by the neonatal team. Skilled, Multidisciplinary Team : The staffing of experienced labor and delivery nurses, a 24-hour obstetrician and certified nurse midwife, and obstetrical technologists ensures that the team is equipped to handle high-risk deliveries. The backup maternal-fetal medicine physician and the availability of pediatric specialists enable a coordinated approach to addressing both maternal and fetal care needs. The emotional and psychological support for these mothers and families is as important as the physical care provided. Job Title/Position : Hospice On Call RN Reports To : Clinical Director JOB DESCRIPTION SUMMARY : The On-Call Hospice RN is responsible for responding to all of the Hospice patient census and their families, as well as handling any referrals or community inquiries outside of office hours (nights, weekends and holidays). The on-call nurse will provide urgent clinical and psychosocial interventions for patients and their families, skilled pain and symptom management and collaboration with providers within and outside of the hospice team to maximize positive patient outcomes. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES : Consistent availability by telephone and email during assigned shifts. Conducts urgent physical and psychosocial assessments of the patient and family during after hours and determines appropriate interventions. Provides and maintains a safe environment for the patient. Provides professional nursing care by utilizing all elements of the nursing process. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and family regarding diagnosis, medications, treatments and progress. Documents patient care thoroughly and timely. Job Summary Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation.
