Position Summary Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards. Common surgical patients include those undergoing Whipple procedures, esophagectomies, gastrostomies, and abdominal aortic aneurysms. The unit is staffed with registered nurses, critical care technicians, trauma surgery nurse practitioners, resident physicians, and attending trauma surgeons. Staff also work closely with respiratory therapists and specialized surgical groups such as orthopedics and neurosurgery. Overview Kelemen 10 is a medical-surgical unit that cares for patients who have various needs and disease processes. Both North and South 10 have Telemetry capabilities for our patients. Each side houses 36 patients to a maximum of 72 patients. South 10 has a 12-bed Intermediate Care Unit embedded within the 36-bed unit. INCU has critically care-trained RNs who support the patient's needs and care for ventilated patients. South 10 also has 2 negative pressure isolation rooms with an anti-room that is used when needed for specific isolation needs. We do multidisciplinary rounds Monday – Friday. This involves the patient in their care and allows the team members to collaborate with the patient to give them the best possible care and appropriate discharge to home or other facilities. We have a pharmacist who has an office on the unit to assist the RNs when needed to help avoid delay of care to any patient. Education Requirements Graduate of NLN accredited School of Nursing. License/Certification Requirements Current NJ-RN License (Registered Nurse). Current NJ-BLS Certification (Basic Life Support). Certain specialties may require advanced certification (i.e. ACLS, NALS, PALS).

A Brief Overview This is a full-time, 36 weekly hour rotating shifts position at the Philadelphia campus . Pediatric Critical care, PACU, Radiology experience, IV skills, are preferred. PALS and ACLS certifications are required. The Clinical Nurse practices at the level of an advanced beginner to expert practitioner of professional nursing in accordance with, and guided by the CHOP Nursing Professional Practice Model. The nurse at this level demonstrates competence in caring for patients and families and progresses to caring for those with the most complex problems/highest acuity in the designated clinical area. Provides age/developmentally appropriate care. Care is delivered through RN leadership for patient care within the framework of family-centered care. What you will do Clinical Practice Follows the nursing process to deliver safe, compassionate, and culturally effective care. Gathers and recognizes/synthesizes assessment data to determine the patient/family plan of care, pertinent data to be shared with care team, and tasks to be delegated. Recognizes and/or seeks resources to recognize clinical changes that are indicative of patient deterioration; utilizes the CAT team when appropriate. Collaborates with the patient, family and multidisciplinary team to coordinate all facets of patient care. Develops, implements and/or revises the patient’s plan of care on admission and during the course of the hospitalization. Plans of care are outcomes focused and include goals as well as required interventions. Plans are developed in partnership with the patient, family, and the interdisciplinary care team. Care is coordinated across the continuum – understands and utilizes case management resources.
