Registered Nurse-OBGYN-Fertility-Center for Reproductive Health

Job Description Under general supervision, guided by the policies and procedures of the Center for Reproductive Health (CRH) and UCSF Ambulatory Services, and in collaboration with the physician staff, the Clinical Nurse role crosses care settings to promote health and healing for a specific patient population. The primary focus of the role is to improve access, timeliness to treatment and care coordination within the healthcare organization. The Clinical Nurse performs established nursing procedures using clinical knowledge to care for patients. The Clinical Nurse collaborates with physicians and other members of the multidisciplinary team in providing excellent clinical care to all patients attending CRH for medical services and treatment. The Clinical Nurse consistently projects a professional and courteous demeanor. The Clinical Nurse is a licensed registered professional nurse who is a clinical expert and actively promotes the delivery of evidenced-based nursing interventions, demonstrates competence in the management of patient flow and daily clinic operations and possesses expertise in the specialty nursing area and specific patient population of REI (Reproductive Endocrinology and Infertility). The Clinical Nurse provides expert nursing care to all patients who are receiving assisted and routine reproductive techniques and services. These duties include in-person consultations with patients and/or their partners throughout the care continuum; patient treatment and medication instruction; EMR charting and documentation, preparing treatment schedules; assessment, management, and monitoring of patient medication needs; assisting with various clinical procedures; efficiently communicating all patient treatment and scheduling needs with the patient and the multidisciplinary team involved in their care; triaging patient phone calls and coordinating patient care between the various practice teams. The Clinical Nurse will assess patient needs and in collaboration with physicians, plan appropriate treatment interventions and evaluate patient response to treatments. The Clinical Nurse will work collaboratively with the multidisciplinary team comprised but not limited to: physicians, fellows, medical assistants, Practice Coordinators, front desk staff, billing staff, Patient Financial Navigators, anesthesiologists, embryologists, embryology and lab team members, procedure nurse team members and administration. The Clinical Nurse shall comply with the medical center house and telephone standards and EMR standards. The Clinical Nurse will assess health needs and perform functions specific to routine and assisted reproductive techniques including but not limited to: andrology, gynecological treatments, in vitro Fertilization (IVF) procedures, fertility preservation patients (oncology and gender transition), PCOS (poly-cystic ovarian syndrome), endometriosis, and male urology patients (micro-surgical sperm extraction). The Clinical Nurse will assess patient needs and develop appropriate plans of care according to nursing practice standards as well as evaluating and participating in care for patients/couples during their reproductive health treatments, office visits and IVF cycles. S/he will utilize specialized knowledge and skills for evaluation and teaching within this patient population, and may act as a resource person within the organization and/or community. Indicate date photograph was taken. • Photograph all residents with any skin problems and with stage 2, 3, and 4 pressure ulcers upon discharge, except in an emergency transfer. • Ensure that all skin problems and pressure ulcers are documented on the resident’s Plan of Care. • Perform a weekly skin assessment on all residents and submit findings to the Administrator, Director of Nurses, MDS Coordinator, Dietary Consultant and Physical Therapist. • Document skin assessment findings during weekly assessment on weekly Nurse’s Skin Wound Progress Report form, inform physician as needed and enter on resident’s Plan of Care. • Notify attending physician and Director of Nursing Services promptly of any deterioration of any skin problem and non-response to treatment. • Notify Dietary Consultant promptly on any need for dietary intervention. • Notify Physical Therapist promptly on any need for wound management. • Transcribe and document treatment orders on treatment sheet. • Audit and check treatment sheet recaps on a monthly basis. • Audit missing documentation on treatment sheet at the end of each month. • Order medication and treatment from pharmacy as prescribed by the attending physician. • Consult with skin care consultant regarding need to initiate or discontinue special bed. • Responsible for daily charges of equipment and supplies. • Review monthly charges before submission to Business Office. • Monitor oxygen supply. • Check and request supplies weekly from central supply personnel. • Change drainage bags weekly and PRN, or as per facility policy and procedure. • Responsible for taking cultures as ordered by the attending physician. • Attend in-service classes, meetings and seminars as required. • Make monthly rounds with skin care consultant. • Keep treatment carts clean at all times. • Notify Director of Nursing Services promptly of any skin problem that developed in the facility. • Others as may become necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator.
